sore and painful toes

An friend (95) in general good health still driving etc., has no doubt an arthritis problem in his feet, which is accepted. However the toes are very painful most of the time, but despite paracetamol, wakes at night with bad pain and stabbing pain in the toes. The Doctor has prescribed stronger pain killers, but they are little help and make him feel dizzy, next day. Any thoughts about this,  please and suggestions for helping with the pain at night?
Perhaps someone has had a  similar experience with an encouraging result.

He should have his blood tested to see if it something more than just osteoarthritis (arthritis due to age and use).  Has he been tested for gout?  What else?  If it is gout he should not be taking aspririn, alcohol or organ meats (there are other foods too that he can google).  There are meds that can help if this is gout.
