Sensory overload and meltdowns

My daughter is still doing really good at school.  It is to the point that her teacher says she does not notice any of her symptoms.  I am really happy she is adjusting and doing well at school.  
She still has meltdowns and sensory overload at home.  Usually starts as soon as we get home.  It makes me feel so bad that she has so many problems when she gets home.  Could her school do something to let alleviate her sensory issues so she won't be so overwhelmed at home?  Do you have any suggestions on how to make the transition from school to home easier for her?  
Also, I have tried all types of discipline for her and none works.  What could I do differently?  Any suggestions?  Also is there any one else who has this problem with their high functioning autistic child?

My daughter was diagnosed with high functioning autism about two weeks ago.
Her sensory issues or meldowns, transition, sensory seeker, etc.

I am sorry but you have not specified what iisues your daughter is having and it is unclear what you mean by "sensory issues".  If you could be more specific perhaps we could provide some useful information.
