ANA Speckled Pattern, Liver BX neg

HI

I had an ANA/IFA result of 1:1280 Speckled Pattern followed by a neg.liver biopsy. My actin, serum ferritan, M2 and alt also came back high. I had a neg C3 and 4, rheumatoid assay, and a few other tests. Still told I may have an auto immune issue and need follow up.  I haven't followed up...

thoughts?

You should follow up. You have a high ANA with a defined pattern which is associated with the diseases Niko listed above. When you mentioned neg C3 & C4 did you it was normal? If it low this is also an indication of active autoimmune activity.

Hmm, I'm  a little surprised because of your high ANA titre.
On the other hand,it's possible that nothing specific came out of the blood tests, which makes sense, since as I mentioned already that your ANA pattern, is the least specific of all patterns.

I must forwarn you that some of these suspected conditions which were ruled out, are usually very difficult to diagnose, specially when not all labs are indicative of them.
Which means that should you be experiencing some of the symptoms, you could have a seronegative autoimmune disease!

The high M2 if relating to mitral second sound, should be evaluated.

For liver issues, you would need to see a Hepatologist.
A Nephrologist is the specialist for kidney issues.
I did very well in Medical terminology because of my background in
Greco-Roman Linguistic studies, lol!

I saw a Rheumatologist that did some bloodwork and ruled out some of the initial Immune issues like the above you mentioned.  I standard blood studies were fine.

I haven't seen a Cardiologist or Pulm. Was thinking a Nephrologist to evaluate my liver further?

Did you mean high Anti-Actin Antibodies?
Have you ruled out iron overload?
Is the M2 relating to mitral second sound and if yes, did you get an opinion of a pulmonologist or cardiologist?
Your high ANA titre points to some autoimmune condition.
The speckled pattern is the most common and least specific and
is associated with :
Systemic Lupus Erythematosus
Mixed Connective Tissue Disease
Scleroderma
Sjogren's Syndrome.
Further evaluation is definitely necessary at this point.
