I am a 50 yr old female,  average height/weight,  Extensive family history of Autoimmun disorders (Both my father (rheumatoid arthritis, pulmonary fibrosis, sjorgrens, crohns, Reynaud's...) brother (scleraderma,  and both my adult daughters have lupus.

I have fatigue, sob on exertion (normal spirometry with a couple volumes on the low side) Positive ANA 1:80 Speckled, Elevated ESR (25, but hx of elevated up to 77)  Positive Von Willenbrand Factor, Low INR,  diffuse lesions in the corona radiata and centrum semioval with T2 elongation, Esophageal spasms,  Right sided tremor,  "electric zings" from head down arms, occasional muscle spasms throughout body  I also have fine inspiratory crackles in both bases.   Any thoughts on this before my appointment with the rheumatologist?

Get checked for Celiac disease, which can trigger other autoimmune disorders.  

Your family needs to try low-dose naltrexone, which would have to be prescribed and most docs don't know about it for autoimmune disorders.  A compounding pharmacy has to mix it for you, but it is not very expensive.  Around $40 a month.

Most patients with Interstitial Lung Disease (ILD) have bilateral inspiratory fine crackles which usually are most prominent at the lung bases.  ILD is a well known complication of various connective tissues diseases with most commonly implicated disorders are scleroderma, rheumatoid arthritis, Sjögren's syndrome, polymyositis or dermatomyositis, and systemic lupus erythematosus. Prolonged ILD may result in pulmonary fibrosis.
