Lower back pain only in morning

I wake every morning with horrible pain in lower back (kidney area) that radiates to the front of stomach (rib area).  I have had battery of tests and found nothing.  I am only able to sleep 5-6 hours before pain wakes me, also unable to take a deep breath until I get out of bed (very painful just to roll over), once I stand and get out of bed the pain almost immediately starts to ease.  After about 20 minutes of walking around and after first morning void, pain disappears for remainder of day.
Could this be a pinched nerve or nerve compression?  Had MRI of lower lumbar which showed normal...chiro recommends stretching excersises, something serious has to be wrong with this kind of pain.  If anyone has any ideas it would be GREATLY appreciated!!

I know this is super late but I am SO glad I found this thread!
For months I’ve been fighting with this mysterious pain that absolutely NO doctor can diagnose…besides saying I’m just “anxious”.

Here’s my story in short:

July 2021 I woke up in the middle of the night for 3 nights straight with searing pain in my mid-low back. Immediately rose to go to the bathroom as I thought I had to pee. Walking to the bathroom was enough to make it stop but the pain has since the. never really left.
I have ZERO pain like that during the day and it bugs really only when I sleep.
I’ve been to DOZENS of GPS, a back specialist, a GI doctor, a Neurologist. I had ESR, CRP, ANA tests done. Also did CBC, liver/kidney panel, urine tests. I had back x rays, and 3 back MRIs. Also abdominal/pelvic CT scan. I’ve suffered from a really unpleasant bout of bilateral meralgia paresthetica back in Nov and therefore had brain stem MRI AND EMG done.( this was because a close family member of mine suffers from a neurological disorder that could be hereditary so they said it counted as history).

I did SO.MANY.TESTS and in the meantime had to also struggle with my GPS giving up on me because they couldn’t find anything particularly wrong (besides a tiny gallstone).
I did PT for months, I went to a (great!) chiropractor for more months.

My pain is now pretty much every morning. I can usually go back to sleep and turn in bed and it’s gone.

I am otherwise healthy besides suffering from anxiety/depression (that has bee. in control by means of regular psychotherapy for years…even way before this pain started).

I am now really scared and sad it could be a very early form of Ankylosing Spondylitis, as many of you ended up writing here.

I hope to finally get the rheumatologist referral I’d SO much asked.
It’s crazy to think that I’m totally frightened of going to sleep now and that I need to fight with my doctors to help me feel better but I’ll do whatever it takes because it’s ME who’s waking up with this nonsense pain every morning no matter what I do.

I am 31, female. Sit a lot for work reasons (but not much more than my friends of the same age who don’t have this pain). But I do work out 5 days a week and enjoy hiking in the weekends.

Unfortunately, I read AS is highly underdiagnosed and I am really left here wondering if most of us are really suffering from this w/o proper treatment because “all tests are ok” (AS is mostly seronegative so all blood work could come back within standard range even if the disease is active).

Praying with all my heart this is NOT it, although I wonder what else it could be at this point.
A lot of absolutely great suggestions here! Thanks again for making this and sorry for the long post. I’ve really never seen a thread like this before and it resonates so much with the mysterious pain I’m experiencing!

I have severe back pain at diffrent points sumtimes in full back .i cannot move while facing this chronic pain cannot getup faster .i can"t change my side whil sleeling quicy . I am not able to stand straight for min 1p0 mins because of pain . It feels like my back will be burst unbearabl pain sumtym pinching sometime inflamatikn cant even move ut after got up in morning it vanished as i started my daily routine it only happens when lay on bed and try to get up  back help mee for soln i m much worried as i get married within couple of weeks

I'm not sure if this suggestion has already been mentioned. I skimmed over the 200-something messages and saw a lot of references to tumors and disk-related injuries but none related to your stomach. Your symptoms sound exactly the same as mine and I was told it had to do with having Irritable Bowel Syndrome. There are lots of causes to IBS. Mine is related to anxiety but other common sources are caffeine, sugar, high fat content, acid-heavy foods (like tomato sauce). My doctor put me on a generic form of Zoloft and that immensely decreased the amount of times in would wake up with that awful radiating back pain. I've also tried anti-spasmatics and increasing my fiber intake. Both helped, the anti-spasmatic you have to take 4 times a day, though, which i'm not great at.

I have similar issues but eventually it went away. I guess for my case it's because I'm using inappropriate mattress but since I got the right one for me the pain is now gone.

I MAY HAVE THE ANSWER!

I am a nearly qualified nutritionist.

I get that alot of you many genuinely have musculo/skeletal problems, but for those of you that don't, your problem is quite likely to be food related.
I have suffered with extreme pain around my lower back/kidney area first thing in the morning for years. It would be painful to breath in and felt like someone had punched me in the deepest depths of my gut/back.
I also used to suffer with extreme migraines, constipation and horrendous period pains. The initial symptoms would only go away on rising and urination. But the back ache would often continue throughout the day.
Due to another health issue, someone recommended that I cut out gluten, and miraculously, all my symptoms went away. Unfortunately due to eating it for years, it has caused my immune system to over react which has left me quite sensitive to several foods, hence going down the route of Nutrition, but this has really helped me define what does what to me and avoid them.

As I'm sure you all know, many people have gastrointestinal problems from eating gluten, but I just wanted to share my recent finding connecting gluten to kidney problems. This completely makes sense with the symptoms related to lower back ache for me.

This is what happening to me, I had to buy a set of heated rollers that I use for about 10 minutes.  Once  I do this and stay up it seems to be fine.  I had a Hydrocele Operation it seem to help it felt fine but only lasted about 2 weeks and it came back.  Dr. gave me 2 shots, went to chiropractor many sessions didn't seem to help me at all before I had operation. 6/3/16 6:56am  ASM.

I am researching on line because I have SEVERE back pain in morning if I sleep in bed no matter what position I try.  I have been sleeping in Loveseat with my upper body elevated and 2 large pillows keeping my upper legs elevated so I am sleeping in "V" shape.  This is only position I can sleep in to be able to get up in morning with less pain.  I saw your situation is much like mine and wondered if you have learned anything else since your post several years ago that might help me.  I have been told to change mattress which I have tried several and nothing changes.  I have had injections with no relief.

Had the very same problem till recently I found out the reason: if I open up my thigh apart deliberately or not (there's some sound on the bone), I will have the pain the next day. Once I controlled my thigh position and not to twist my spine, I am perfectly fine now.

I had this problem and I found a solution that seems to work for me.  I am 69 years old with serious deterioration in my lower spine.  One of my disks is entirely gone and the xray looks something like a train wreck.  About two years ago I developed a serious pain in my hip, which was apparently caused by a nerve being pinched in my spine.  After some physical therapy and about a year of various back exercises, I overcame this problem, but then I started having serious morning back pain.  The pain would get much better or go away entirely after about 30 to 60 minutes after I got up and about.  The pain was on each side of my back, just above the hip bone.  Sometimes it was so bad that I could barely get out of bed.  I noticed that a back exercise called the "bridge" seemed to affect the same area that was giving me trouble.  I added this exercise to my existing back exercise routine.  After about two months of this, the morning back pain is almost all gone!  The exercise I am referring to is to lay on your back with knees bent and lift your hips off the floor as far as possible and hold it for 15 to 30 seconds.  I also lift my hips up repetitively 10 to 20 times to "work" the muscles involved.  I try to do this 3 times each day.  From the physics of this, I suspect that this exercise compresses the spine, which I do not want.  It is compressed enough already.  So I alternate this exercise with other back exercises which stretch the spine.  Don't know if this will help anybody, but it worked for me.

It's so reassuring to know others have experienced this and I'm not going mad! I've been waking in a morn and during the night with what can only be described as an aching waist/lower back which disappears after about 5 mins. It's so painful I can't turn over. I've had it for a couple of years on and off & always put it down to mattresses, period pain or as I am prone to water infections/cystitis that it's linked to these. But I'm sure it isn't as have experienced kidney pain which feels much different. I am fit and healthy with a varied diet  having engaged in running, the gym and dancing with a lack of exercise over the last few months due to lack of time. Ii get test results back tomorrow to rule out traces of a urine infection as this pain has been so bad this week & even surfaced in the afternoon when out walking I decided to see the doctor. Hopefully he will get to the bottom of it but I feel it's a minefield trying to pinpoint the exact cause & guess everyone feels abit like that. Good luck in finding a cure everyone. This is a really useful forum. :-)

I have had this pain in my back at the side for a while. It's only when I first wake up and when I get up for a bit it goes completely away. If I stay in bed I cannot get comfortable.  I go to a chiropractor and she helps tremendously.  She says it is my sacroiliac. Somehow the muscle tendon has "come off tract".  I have spinal stenosis and sciatica for years as well. I've just started back seeing her the chiropractor. It usually takes several visits for me to feel a good amount of relief. I  I'm 56 years old and have led a very active life. I've also fallen many, many times but some of my problems are hereditary.  I hope this helps someone.

Don't know if this helps but I was doing this for years while having tests too.   They finally ordered a sonogram of the kidney area while I was laying down and the radiologist saw what was happening.  I had 3 cysts on my right ovary.....one was ruptured and one was leaking.  While laying down the fluid runs into the gutter area around the kidneys causing severe spasms and pain in the kidneys and lower back and hip areas.  It stops as soon as I would rise or sit up. I only got @ 4 hours of sleep each night.  Doctor prescribed a pain medicine for the urinary tract area and said my body would eventually take care of the ruptured cysts.   I was continually plagued with them until I started menopause.  I'm now 48 and haven't had to take anything for them in a few years.  I did have to have one scraped off my left ovary 6 years ago due to the extreme pain.

I have the same problem:
Only on the morning pan and afer a bit of walk around the house it all goes

I have read quite a bit around and i think is because of gas.

Also if i have conticipation my pain is even larger...

So my opinion is the diet and gas

Thank you for your response philnoir. I have seen my primary care physician and she has done some x-rays on 8/13 (F/U 8/28) the thing with my back pain is, it wakes me up with horrible lower body pain. After I've moved around for a little bit I'm fine, no pain at all for the rest of the day. Even lifting and pulling 200 lb. Patients, do not bother my back. My Dr. has precribed me cyclobenzaprine and tramodol but I've only taken it one time before bedtime. And it's baffling that this back pain only happens in the morning, usually waking me up, and there are some mornings I don't have this pain. I have tried different mattresses, even have slept on the floor, to no avail. I'll update when I get results ;-)

I usually don't reply to tail-end Charlies -- people who add a comment to a thread that's almost as old as I am, and has 300 posts. I just don't have the time to read back through the comments that say "I have just what you described."

But let me share a little information.

LBP (low back pain) is the #1 cause of medical visits in a country where 100 million people live with daily intractable pain. Pain is America's worse epidemic, ever, and LBP is a leading part.

There's many reasons for LBP, but not all are idiopathic -- of unknown cause.

Your problem could be anything from a bad bed and sleeping posture, to severe spine disease.

Strengthening your core muscles -- the deep low layer muscles, like the ileopsoas, that support your spine. There are specific exercises you can do every day to build these muscles -- many people see a remarkable decrease in pain by doing so.

But if that doesn't work, and you are 50 years old (or more), you are likely to have a painful degenerative spine condition that can lead to disc bulges and herniations that put pressure on nerves and cause pain. This condition progresses, causing painful arthritic changes to the joints in the back.

There are treatments for these problems, but if you don't start by seeing your doctor for a history and physical, and some imaging of the lumbar spine, you'll never benefit from them.

You're one of the sharper tacks in the box, so I'm sharing this information with you in the odd chance that you follow up on it and receive some relief.

I've read every single comment under this post. I to suffer from this monster! First a little about myself for comparison.
1. I've had bi-inaugural hernia surgery with mesh (I am allergic to the mesh they used)
2. I suffer from chronic kidney stones since I was 19. I am 42 now
3. I'm severely lactose intolerant
4. I have GERD with small ulcer and inflammation of the stomach due to lactose
5. Diagnosed with anxiety
6. I get Charlie horse in my upper right ribcage when I bend in a certain position.
7. I have plantar fasciitis.
8. I currently take Super B Complex vitamin 1tab daily, slow release iron supplement pill (Slow Fe) 45mg daily, CLA 1000mg 2caps morning 1 cap evening, 500mcg Chromium daily, 10mg zyrtec daily for allergy to mesh.
9. I drink at least 48 oz. Of water per day.
10. I was a dancer for 30 years
11. I'm currently a Nurse Assistant (2 yr veteran). So 3-12 hr. Shifts mostly on my feet, lifting pulling, turning patients
12. My low back pain started 3 yrs ago, and some mornings I don't have it
13. After about 5-6 hrs of sleep I'm awoken BY excruciating/paralyzing pain in my lower back area radiated just above my hips into the lower half of my body.
14. It's takes me at least 5-10 minutes just to roll over to my side and push myself up to sitting on the side of the bed with crippling pain.
15. I don't feel steady at all walking right away as I feel my lower body will break off at any moment the first 30 minutes after I stand.
16. Bending over to pick anything up off the floor the first 45 minutes to an hour after waking up, is impossible.
17. Passing gas seems to immediately relieve the hurtful pressure I feel.
18. Sitting on the toilet involves FALLING back onto the commode with a udder scream.
19. There are some mornings I don't have the pain. More so than not though.
20. I've went thru 5 different mattresses to no avail.
21. I've tried sleeping on the recliner, i was relieved for a few days but episodes came back.
22. I've tried beginners yoga for a week, it helped for a few days than the episodes came back.
23. I've gained 25 lbs in the last 2 years with failed attempts to losing it with strict diet and exercise.

This thread has comforted me in knowing I'm not alone in this. And I have some options to try. I will keep a journal for a few weeks of my diet, my activities, my supplements, and my hours of sleep and time of day I sleep (I work 7pm-7am) so some days I sleep while most people are working.
Things I'm going to try: (and I'm going to give each one below a few weeks so I know the pain won't come back) than I will report back to you all with my findings
1. Epson Salt: 1-2 tsp. In bottle of water every other day for two weeks. My grandmother swears by Epson Salt so I'll try this first!
2. PSOAS stretches: 15 minutes in the morning once my flare up is gone, and 15 minutes at night 1/2 hour before bedtime for two weeks
3. Take a daily prilosec tablet before bed for two weeks
4. Try a gluten free diet (though I'm already lactose free diet, so this one should be easy to do) for two weeks.

I do know that doctors and chiropractors don't know the cause of this just by all of the comments. So I'll avoid all the tests and sessions that you all have tried with no relief. I do believe that this thread there may be an answer and correlation to our problem. This condition, as I'll call it, the "Devils Back" does not discriminate against age, gender, nationality, or fitness level.

I spent hours researching all the same complaints for my husband online as he was very worried & Doctors weren't finding anything to help...his symptoms were: mid-lower back pain, abdominal discomfort(which would wake him every day/night ) heavy feeling in stomach after eating,anxious feeling,tingling in the hands and feet,broken sleep, and brain fogginess...well this all led to a B-12 Deficiency,Magnesium as well, and needing to take a Probiotic daily& remember to take the vitamins at a different interval of the probiotic, because if you take all at once your discomfort will increase...I myself was suffering from the mid-lower back pain, also a skin condition and after much research I chose Dr Clark Cleanse system & slowly our symptoms are resolving...use a B12 Sublingual Quick-Melt so that your body absorbs it completely, research the best option for you & patience is key although my better halfs' sleeping improved within -3-4 days of starting the b-12 & magnesium, Hopefully this helps some of you with resolving your pain & discomfort, also my pain also diminished in the mornings within the 4th day of starting the cleanse....My best to all of you

I've had the same problem. anyways, it's a funny looking pillow that wraps around your neck and you fit it in between your legs. Sleeping on my sides with it or on my back with the bottom part under my knees have completely abated the pains i would wake up with. Before using her pregnancy pillow, I tried just using a normal pillow between my legs, and that technique did not work as well. It was barely a little bit better than having no pillow at all, which is the worst, and makes my back act up in the morning. Anyways, the pregancy pillow has been a blessing to me so I'd recommend it if you can find it for sale somewhere, craigslist maybe. It's worth trying out to see if it helps you. Might be worth it paying full price too, because like I said it has completely removed the pain feelings in the morning if I remain using the pillow all night.

Mine started after I was diagnosed with Graves disease (a problem where the thyroid is overactive)  Before that I never had this awful pain. Like many of you the pain is in the kidney area and is ferocious most mornings. It is hard to inhale or exhale without crying out. My thyroid levels are now back to normal (for now) but I have been left with this awful pain every morning. Never had a back injury and do not have IBS so only rational answer is linked to Graves disease for me. Sick to death of dreading waking up and miss being able to have a lie in. The pain can sometimes last more than an hour and I am still convinced it is kidney-related as going for a wee helps to reduce the chronic pain by some degrees though the ache still continues

I first experienced this problem in my late teens, early twenties. It lasted about 6-7 years then just Disappeared..!
          Myself and my now x-girlfriend had our first child when I was 18 years of age. As most of you guys will know with Children comes a great responsibility!!! I wanted to be all I could be for both my now x-girlfriend and Daughter Stephanie, the true light of my life..... I moved my girlfriend out from her parents house when she'd fallen pregnant with Steph and we moved in to our own two bed flat.. Life was wonderful. I started working 2 jobs to pay for all things we or anyone could ever need. The flat, Car, holiday once a year then the back aches started lower back. The dullest pain you could imagine I'd awake every morning with this serious pain to my lower back. I put it down to being over worked and very under slept. As I previously stated I was working 2 jobs a 9-5 @ Senate Electrical and a 8pm-2am @ Domino's Pizza. So I never seen a GP about this I just assumed it would go. 6 or 7 years after the start of this pain it did subside and I was free from morning pain. (Phew what a relief). Not...!
          At the age of 38 now running my own construction company, on a weekend in Bath with a different x-girlfriend lol.. Outside of a hotel in Bath City centre on a fabulous summers day drinking cocktails after are walk around the "Hot Spa's" my Aorta suddenly decides to split in 2. I later found out that for most people who suffer this there dead before they hit the floor. I was 7 hours in a hospital drifting in and out of Consciousness knowing I was dying. I could hear the nurses telling my x girlfriend to say her goodbyes as they thought I'd only 5-10 minutes left of life. After 7 hours there CT scan showed that I had a Type A, Aortic aneurysm and was in fact dying. I hung on in there how I'll never know. A further 3-4 hours past during this time they had to move me by road ambulance from Bath hospital which was too small to perform the operation I needed to nearby Bristol Hospital.
          Bristol hospital was equipped to perform the operation I needed to save my life. 10hours 23minutes in the operation commenced under senior thoracic surgeon Jonathon Hutter. My body was frozen, yes frozen so they could reduce the blood flow by basically turning it to ice.! The operation was a success. However on one of there scans they couldn't find one of my kidneys and the remaining one I did have was only 30% working lol out of the frying pan and in to the fire. I was then after spending a further 2 weeks in Bristol hospital shipped yet again by road ambulance this time to the Renal care unit of the Royal Free Hospital, London, NW3. After 2 weeks there my kidney recovered well enough for me to return home now Girlfriendless...! Lol, Anyhow if in my late teens early twenties I seen a GP or went to the hospital with the pain, maybe I'd still have both Kidneys.... Who knows.?
Now 4 years have passed in May this year since my life saving operation in May 2011.
          I'm back to full fitness all is well with the exception that over the past 3 weeks that same lower back pain I experienced in my late teens, early twenties is now back.....

I have been suffering from this problem for 15 years, it was so bad originally that i could not walk it a straight position for a couple of hours after waking which caused me to get bursitis in my thigh so I had pain at night from the back condition & pain on activity from the bursitis. The pain on waking in night and on getting up felt like my lower back, hips & pelvis was paralysed with pain, turning over to a position that eased it was agony & I would cry out but it would help once I had turned. On waking it is hard to sit (even getting on the loo is agony), lean forward or bend to get dressed etc

Had numerous tests, steroid injections & pain management and six years ago the pain just went, no explanation no reason  it just disappeared.

A couple of years ago I started to get the pain at night again, not as bad and manageable, I was really stressed at the time and I could feel I was holding my muscles really tight on waking with the pain, it felt like a tight coil. I found that using my mind to try to relax the muscles really helped but its agony to do, the best way is a little at a time, wait for pain to ease then relax deeper. I started swimming every day, just a length at first but got up to 20 lengths a day within a month & again the pain went.

In December my best friend died from Bowel cancer and within days the pain came back with a vengeance, I know Dr's would jump in to say its psychological but the truth is it is in my case its muscular that has been caused by stress. When we are stressed we hold our muscles really tense & I am 100% sure this is the cause of my problem. The same would happen with bad posture, sitting to long or any other reason for muscle stiffness so there is no 1 reason.

I am controlling mine by taking 2 paracetamol & 2 ibuprofen on waking, waiting 15 mins & then doing muscle relaxing exercises (its 2 painful to do without the tablets). As soon as I am able to relax the muscles the pain goes within 15 mins & I am fine all day.

I think this is a two fold problem, firstly you need to find the reason for the muscle tensing whilst sleeping & work on that, secondly u have to take measures to get those muscles to relax (which is agony but well worth it). I have found that knowing how to get rid of the pain quickly when waking makes it bearable.

Don't spend too long in bed, the longer the muscles are tense the worse the pain. I try to stay in bed for a maximum of 6 hours & then have a power nap for an hour when returning from work to make sure I don't get overtired, this really works as the hour never leads to pain.

Finally I want to say the more you worry about this problem the worse it gets as you will naturally be more tense when u are worried. After living with this for 15 years I know its not serious (although painful) & that it is probably only a small amount of cases caused by mattresses or sleeping positions. Please believe me that it is caused by over tensing of the muscles caused by a huge spectrum of reasons & the only cure is to get those muscles to relax.

Do you have a congenital twisted spinal column ? And Did you
try to twist your spinal column ?

or

Do you have a normal spinal column ? And you have tried to
twist your spinal column Strongly to be more flexible?

if this is happen

the solution is that

you have to accept your original spine

  try to go back to your congenital design

so

  you have to adjust your mattress to fit with your original
column's design by  slight elevate somewhere under your
back

put slight small tissue layers

I tried this and this pain disappeared

I have had lower back pain every morning for past ten years. 6 months ago my wife suggested I ate to much bread as she had similar problems at times of high bread consumption. I stopped eating beard and 2 or three days later pain had gone totally. been good since with only two relapses of pain due to bread eating . hope this helps . very interesting .
