severe headaches

i am a 34 yr old male that has severe headaches that can last for a week at a time.  the pain generally comes from the back of my neck, and it gets sore and sensitive behind my right ear.  i can get light sensitive but not always and can get sick.  no tylenol, ibuprofen or any other otc meds seem to help, i cant sleep because they hurt so bad, and i can get little relief with sleeping with a heating pad under my head.  if you apply pressure about an inch from my ear (closer to the back of my neck that the jaw line) it can dull the pain.  i usually have at least ona month.  any suggestions on what can be causing this?
