Chances of pregnancy after Plan-B

Hello Doctor,

I was wondering if you can help me better understand the risks and variables associated with a pregnancy scare my girlfriend and I are currently having. To summarize: we had unprotected sex  a week ago on July 17. We used the withdraw/coitus interrupts method. Within an hour of intercourse, she also took Plan B One Step. There were no other instances of unprotected sex this month. Our concerns are two-fold: Firstly, we read that there are recent studies challenging the efficacy of Plan-B one-step after ovulation. In our case, it is difficult to guess when ovulation occurred (if at all) because she suffers from PCOS. In fact, this condition has caused her to miss her menstrual period for the past two months. Also, I saw in related literature that new research calls into question the efficacy of Plan-B in overweight women, which she is. In trying to ***** our risk for unwanted pregnancy, things are further clouded for me by the fact that when I recently had a sperm analysis done, the results showed low motility and low-volume due to a condition that my doctor calls "retrograde ejaculation". We are anxious because this would be a very difficult time for us to have a child. Could you help us gauge our risk for unintended pregnancy in this instance? Thank you

hi philos I just wanted to share my story...I was on the poll due to irregular periods they would come every 4-6 months some times longer.. Anyways I never missed a pill and took it the same time everyday... When I decided to have sex for the first time I had taken the plan b pill right after and than 72 hours later line the prescription said... Well  12 weeks later I found out I was pregnant... And I was still on the pill and had taken the an b...idk if it was hit because I was irregular or not but the plan b didn't work for me.. I was 16 but now I have an amazing 7 yr old

You may have also read recent studies which challenge the old belief that there is sperm in the pre-ejaculate. All things considered, I think your risk is quite low, never the less, if a patient of mine had a similar story, I would advise her to do at least one pregnancy test unless she had a normal menstural period within 7-10 days.  
One thing to consider is that oral contraceptives provide good cycle control, reduce the risk of endometrial cancer, and prevent most conceptions if properly used.
