High MPV 12.9

Hi

I went to the Dr for unexplained weight loss of 12 pounds in a three month period, severe fatigue, and weakness. My over all health is good except for liver fibrosis which is stable. I dont have diabetes, or heart problems. All of my blood work was normal except the MVP, which has never been high. Previous reading going back to three years have all been stable between 8.8 to 9. My platelet count is normal 219. I also do not have iron defficiency, or any other factor that could play a role. I also do not have thyroid disease, that has been ruled out.

thanks

Hi!
High platelet count can be due to an inflammatory condition like rheumatoid arthritis or due to anemia or excessive alcohol drinking. It is also increased in some bone marrow disorders like myeloproliferative disorders (polycythemia vera, essential thrombocytosis, idiopathic myelofibrosis and chronic myeloid leukemia). All known causes like myeloproliferative disorders of bone marrow, essential thrombocytosis, chronic myeloid leukemia, rheumatoid arthritis etc have to be looked into, and if need be bone marrow biopsy performed. If these are ruled out then other rare causes need to be looked into. One is an absent spleen or a poor functioning spleen. A rare condition known as Kawasaki's disease, which occurs as a result of inflammation of the arteries is another possibility.
The high vit B12 can be due to taking vitamin B supplements. However vitamin B12 can rise in hypereosinophilic syndrome (raised eosinophil count), chronic myelogeneous leukemia, promyelocytic leukemia, and polycythemia vera. Other causes are cirrhosis, acute hepatitis, hepatocellular carcinoma and liver metastatsis due to other cancers.
Similarly low vitamin K can be due to diet deficient in vitamin K or due to a liver disease or inflammatory bowel disease or due to eating disorders or because of certain drugs such as anticoagulants.
Considering that platelet count is high and platelets are abnormal, vit K is low and vit B12 is high, liver diseases and cancer should be investigated for immediately. Please discuss this with your doctor ASAP. Take care!

I went back to the Dr and my mpv is now 13.2 and my platelet morphology was abnormal showing large platelets.  My vitamin B12 is about 1200 points higher than normal and my vitamin K is dangerously low at 0.04.
