Hi,
Fibroglandular density means that the tissue in the breast is dense, making it more difficult to detect certain abnormalities.
It's hard to say what these ill defined lesion detected on your ultrasound represents.
Depending on your BI-RADS score assigned in your Ultrasound report by the radiologist, the surgeon will  examine you and decide which further testing is required and if a biopsy might be needed.
The swelling and pain in your breast could be due when a milk duct is blocked and cannot drain properly.Yes,you should continue to breast feed  by varying the baby's feeding position, massaging the breast and applying heat before feeding. If this does not resolve the problem, then further evaluation is needed in case there is an infection that would need to be treated. Some enlarged lymph nodes could be the consequence of an inflammation or infection somewhere in the body. The fact that central fatty hilum is noted,it's a good sign.
Take care and best wishes...
