What awful news about your young wife. IBC is unfortunately a very aggressive bc disease, and often afflicts younger patients. I do think however, that the Cleveland Clinic is perhaps one of the two top hospitals in the US for cancer care, if not worldwide, - the other is Mayo clinic, but I don't think with your wife's diagnosis they could help any better.
I am rather surprised by some of your wife's stats : being ER+/PR- and HER2- is good. Although when I look at the stats now, your wife's 13/14 positive nodes was not a good prognosis and IBC is known to be very aggressive, especially in younger women. This probably means the cancer had already spread to other organs before she was diagnosed. I had 4/18 positive nodes, so you can put that in perspective, but I had invasive ductal cancer, which is less of a risk than IBC.
One item I don't understand from your post is: "residual cancer, clear margins."
I had clear margins after a lumpectomy, but what does residual cancer" mean? I would ask her doctors.
The fact that she is now in palliative care I guess means they have tried all options. I am so very sad for you, a very caring husband who has taken the time to come on here to post his concerns.

Also, call in other doctors for a second opinion. Always get a second opinion. Did they do a biopsy on the liver spots? They could just be cyst.
I am sorry about your wife. I had breast cancer but was fortunate that it had not spread. Please go to breastcancer.org. Then community support, join the discussion board and scan through the topics there. There are ladies there that are going through what your wife is. They can tell you about other meds that they are on that may work. Or you can post your question here to the doctor on the doctors forum above. We only get 2 questions a day so posting in the early morn is best. There are allot of new and wonderful drugs out that possibly will help your wife. I wish you both the best.
