Paget diease?

I've posted a while back but under woman's health and had no responses to my post however since then more issues been happening so i decided to post here in the B/C forum.

Let me start from the beginning. April 2015 I had noticed a white milky discharge coming from my left nipple. I believe a white milky discharge is normal. I thought maybe I was pregnant and did a few tests but all was negative. I went and did a blood test and was negative aswell. After a Couple months it went away.
About 5-6 months ago I started getting pains in my left nipple, tender, warm, swelling, red, flat flakes? And the milky discharge again. I just kept putting off on it thinking its not a big deal.. However the pain was starting to get to me and it was causing a bruise like feeling from my breast to my armpit. I made an appt w my doctor and she referred me to get a mammogram and ultrasound done, and on my online record it stated that she felt it was warm, slight TTP under armpit and left side of my left breast. I did my mammogram and it came back saying birad 2 scattered fibroglandular destinies, benign clarfications present.. However I was still having pains and my nipple was still swelling red painful itchy on and off. I did a few tests like prolactin and some other hormonal tests. My prolactin level was on the high normal. It was at 30.1 which was the last before not normal and my other hormonal tests were normal. I have felt a wet spot a few times on my shirt. Two weeks ago on Thursday I had milky discharge in one duct and in another duct was blood. I knew my doctor wasn't in the office that day or Fri so I was gotta keep track of any changes and let her know on Monday but a couple hours later I had a email from my dr ( weird out of no where when less expected ) asking how I was feeling and if I am still having the nipple pain and itchy.  I of course responded and let her know that I was but was also having blood discharge. She contacted me that evening 830 saying that she is referring me to a surgical oncologist for paget disease.  That of course got me worried because my grandma had that and had her nipple and areola removed.

However I have a question and not sure if this is related but I noticed a new change in my nipple. I have like a white skin spot on it? Its like my skin is making new skin but don't see no peeling.  Its hard for me to explain. My nipple color originally is a dark color and mostly pigmented? And a spot is not pigmented? Anyone know what I mean? Is this related?

I see a B/S on the 7th.

Thanks

Hi,
I am so sorry things are getting a little complicated and you feel discouraged, but please try to stay calm and understand that what your Oncologist is recommending is not at all unusual. When tests don’t give definite answers, a biopsy procedure is always the best thing to do to know exactly what’s going on and it doesn't necessarily mean that malignancy will be found. Many biopsies results turn out to be benign and I hope the same for you too.
Wishing you all the best and good luck!

Thank you.  Sorry for the late update.  I had went for my Mri on the 11th and got nervous and refused after being put in the machine. I went and tried again on the 25th, while being sedated-was scared again. I did another ultrasound and then had a followup with my oncologist.  I haven't seen any bloody discharge in a couple weeks she asked how i was feeling and ive told her good still some pain ( my upper left side been feeling bruised)  and i let her her know about not seeing bloody discharge but the milky one yes. She decided to check and said its not good, that i would need surgery and went on talking to me about cancer. ( she dont think its paget ). Shr want to send it for a biospy. I broke down crying cause i went in with high hopes. I dont know what to expect. ;(

Hi and thanks for the update.
I am pleased to know that your Doctor sent you for a MRI test which can be very useful to clearly identify  any abnormality that could be present, especially if you have dense breast tissue.
Try not to have negative thoughts about what's happening. Just keep your appointment for the MRI as your doctor suggested. This is standard procedure and good medical practice.
Wishing you all the best and hoping that everything will turn out just fine for you.

thought ill update.  I had my doctor appt today.  I've never had an appt w a dr that lasted long. I was in the room for a hour and 50m. We talked, asked questions, I answered questions, we went over my mammogram results and she checked my nipple and breast. She referred me for a MRI. I have an appt Tuesday morning.  I also had a fever that I wasn't aware of and had to have blood work while there. She did see blood and felt something but said I also have dense breats so it's hard. I dont know what to expect on the MRI?

Hi,
Nipple problems could be so many things. It could be hormonal imbalance, eczema, fungal infection etc. But it’s hard for anyone to say exactly what is happening without the benefit of some type of tests, including a biopsy procedure.
“Paget disease of the nipple “is a rare type of cancer that starts similar to a skin problem that can indeed be overlooked by many doctors. Improvement in the skin can occur spontaneously, but this is not a sign that the disease has disappeared. After a few weeks or even months, the symptoms may reappear and show more serious destruction of the skin. At this stage, the symptoms may include tingling, itching, increased sensitivity, burning, and pain. There may also be discharge from the nipple, and the nipple can appear inverted or flattened against the breast. From what I gather from your post, your nipple problem could indicate Paget disease, and I commend your Doctor for advising you to consult a breast Surgeons. A punch biopsy could be performed and the tissue examined under the microscope. By having this procedure done, you and your Doctor will definitely know if cancer is present or not.
I sure hope that you don’t have this disease but just a skin problem that can be resolved with appropriate medication.
Best wishes and good luck!
