What should be post operative treatment for me?

Age 29, Family h/o Ca Brest Mother,
Wide Local excision of L brest lump done on 25.10.2001
Infiltrating Duct Carcinoma Grade III Detected
Modified R.B.score 3+3+2=8
Necrosis is noted.
Desmoplasia is also seen.
ER & PR : Negative.
CerbB2 : Negaive.
Quandrentectomy + L Axill.Disscn was done on 05.11.2001
Histopath Report : No E/o tumour metastasis, Fibrofatty tissue seen.
Apical LN : Reactive Lymph node, No E/o Metastasis.

What should be the post operative treatment for me?
One advise given is only Radiography + Tamoxifen
Other one is Chemotherophy + Radiography & no Tamoxifen
I am confused.

Dear rasika:  You do not mention the size of the tumor, which might suggest whether or not to pursue chemotherapy.  Since you had a lumpectomy, radiotherapy would certainly be recommended.  Chemotherapy might be recommended.  The role of tamoxifen in estrogen receptor negative breast cancer is not well defined.  Tamoxifen may be recommended in an effort to prevent a second primary breast cancer.

I don't know why Tamoxifen is being recommended, if your tumor is ER/PR negative that means is not responsive to hormonal treatment.  Radiation is probably a 'must' in your case (since you didn't have a mastectomy, and your age is younger than 35)

Chemo is always recommended if tumor is greater than 1 cm unless the features of the tumor suggest a slow growing cancer. Necrosis and grade III are unfavorable features.  Be proactive and get as much information as possible, one suggested reading is Dr. Susan Love's breast book.  Good luck with your treatment.
