Update:
i had a close family doctor who does MRI readings give me a quick checkup, he felt the lump right away and turn the lights out and flashed a flashlight up to it, he showed me how u could see where the lump was and that it was see through as compared to the testicle. He says I can get a ultrasound if it will feel better in my mind but he says im ok, I told him ive had the lump for a year maybe longer as i have and its stayed the same size.
Also the other night I kinda was rechecking to see if it growed and i squeezed hard and it seemed to get smaller and caused a ton of pain. Not sure what im describing is possible. Would a cyst show up on a flashlight scan, I tried to do it but cant really tell.
I want to note that I've think Ive had this "third" testicle for about a year maybe longer, i did discuss it once before with my friend's father who is a doctor. He felt it and dismissed it quickly because it was so small and moveable and said keep a eye on it, its stayed the same for about a year and I haven't seen any growth or loss of pain when i squeeze my testicles etc.
Hey man. I have the same exact thing on my left testicle. I've had it for years! This is probably an epididymal cyst. Unless the little lump grows or it hurts, I'm guessing your okay. I checked for epidiymal cyst and it sounds exactly like what we have and it said they are better left alone. I've never gotten medical attention and I'm fine. I'm sure you'll be okay unless, like I said, it grows or hurts.

Based on your description, the lump could be due to any of the conditions i mentioned.
It is most likely in the epididymis or spermatic cord since it is freely movable. However, the other conditions mentioned could also present as such if still early in the course of the disease.
I strongly recommend consult with your physician.

based on the speration and it not being directly on the testicle what would you think it is? I can slide it around but it is attached to something. If it were cancer would i be able to slide it around? I can slide it around so it touches the testicle as well.

Do you have palpable inguinal lymph nodes?
Did you have genitourinary tract infection or trauma recently?
Lump in the scrotum could be due to hydrocele, hematocele, epididymal cyst, neoplasm, or spermatocele.
I agree with your decision to seek consult with your physician for proper evaluation and management.
