Looking for doctors in New York State to help with Chiari management

I am a 35yo female just officially diagnosed with a symtomatic Chiari 1 malformation. It was 3mm a year and a half ago and is now 6mm. I don't know of any doctors/ specialist in NYS that can help, there is limited information. I live in a very rural community in upstate New York by the Canadian border. Any help or guidance would be greatly appreciated, thank you.
