1 year post op, swelling in neck

My son was decompressed in Dec '12.  He has recently been complaining that his neck/back feels tight.  He said it doesnt hurt bad, and rarely asks for medicine for it.  Last night when he was getting out of the shower I noticed his neck looks like it has a large knot just under his incision.  (Picture posted)  His incision is healed wonderfully, so I am not sure of a couple things.
1.   Does it look normal or should I call his Dr?

2. Which Dr do I call, if any Dr should be called?  His pediatrician?  Neurosurgeon?  Neurologist?

  Hi...is the area hard?.....Does he have good ROM in his neck?.....

Since this was a yr ago that he had surgery, I can not think of nething but possible scar tissue formed....

Deff call the Dr and ask to have it looked at.

That should say he was decompressed in Dec of 2011, not 2012!!
