5 year old behavioral issues

I have a 5 year old little girl who has shown hyper activity ever since she was 1 year old. I could never take her anywhere without her all over the place. She has excellent grades in school but once she has a meltdown it takes her about 30 minutes to get back into routine. I have grounded her, taken away special items and have even used soap in her mouth. She is very distracted when doing after school activities. She will throw fits, in public, and then later call herself "stupid" or "dumb" when feeling remorse. She has a fear of toilets and would much rather pee her pants than use the bathroom at school or anywhere else. Please help. I am in desperate need of advice.

The evaluation should be conducted by a child psychiatrist. Hyperactivity of the sort that is typical of children who display ADHD is a neurological condition and requires a biological treatment (depending on severity) in addition to non-pharmacological interventions. A helpful book to read is Russell Barkley's Taking Charge of ADHD. It is written for parents of children who display ADHD.

We have been working with the counselor at her school and her teacher has been really proactive with her and her issues. I am not sure where I should go with it, a pediatrician or a psychiatrist. Her teacher said she would be quite surprised if it was ADHD so I am wondering just where the hyper activity is coming from. Thank you for your input!

You didn't mention that she has ever been evaluated by a pediatric mental health clincian, so I am assuming such an evaluation has not occurred. It's interesting how often parents overlook specialized evaluation in the areas of emotional and behavioral functioning, when they would never overlook specialized evaluaion for a strictly medical concern. You should arrange an evaluation.
