9 years old bad behavior all the time

My Partner  has an 9 and half yr old son (with diagnoses FASD, and ADHD, he takes 54mg concerta, and 6 pills a day of clonidinem salmon oil, multivitamins)who just seems to not care about anything. It is very upsetting to me that he has no care in the world, his behavior is something I never seen in my live, He desrespect me everyday, or his teachers at school, make jokes of me, he already pull of nives on me.But in front his father he is a perfect child, and when his father is not around he changes,  Nothing we seem to do (all kind of dicipline) even rewarding him seems to make a difference.
The pictures is this:
He lives with me and his adopted father, his adopeted mom lives far way, he see her on holidays,but she talk with him about go to the court to win him, and makes all changes on his medication when he is with her, and She is not on the same page, she changes the routine, food, and does everything he wants.

Can you give any help, please?
Sorry, I know the case is complicated, but any help will be great!
We live in a small community, in Canada, we don't have  resources here, I make my researches at internet, I read books, followed instruction from the Sunny Hill hospital where he did assessment.

The instability and conflict in the family are certainly contributing to his difficulties, and to the extent that the conflict can subside he will show some improvement. He should also be in therapt -medication alone is not a good idea. Check with the hospital about resources to which he can have access. Because he is adopted you likely will not know about the family psychiatric history, but it may well include some mental or emotional disorder. He definitely needs clinical intervention beyond the medication (and you and his father need ongoing guidance about how to manage his behavior and how to manage the family conflict).
