Child regressing - won't use toilet

For he last part of 2 weeks, my 4.5 year old son refuses to move his bowels in the toilet.  He's probably impacted by now.  His doctor told us to give him miralax - that resulted in diarrhea.

It seems almost as if he's forgotten how to tell when he needs to go, or doesn't care. I'm sure it may hurt, but it's probably because he hasn't had an actual bowel movement in nearly 2 weeks.  Just bits in his underwear or pullups or the diapers we use when he's soiled everything else.

I just had him sitting on the toilet for more than 30 minutes - he cried the whole time, even made himself throw up.  Me, I screamed at him.

Need some advice as to how to win this war.

Screaming will only make it worse, I am sympatheitic as I am sure you are worried about him , Perhaps give him the miralax again but only a small amount , also maybe he is over anxious as a lot has been made of it, how about you simply say nothing , and see what happens , give him the medicine and let him get on with it, too many words and too much attention have made him overly worried  ,back off, if he stains his pants its okay ,they'll wash, if you relax he will. Make sure his diet has plenty of roughage and fruits , children at this age are often picky, get a cereal into him every morning , Orange juice will help.
