Help

I am the mother of a 7yr old boy, and a 11month old girl, Now ive always had problems with my son with not listning and being distructive, and mouthy, He is always getting into trouble for these issues, now that he is 7 he is very disresctful, and nothing is good enough, I send him to his room, and he laughs at me, and goes up for 1 minute then comes down, and when i tell him to go back up he wont go, I have a heartcondition so it takes a toll on me when im having to get upset cause he wont listin, Ive taken parenting classes to learn how to be consistant, but I didnt learn much, ive used everything they had mentioned, Ive asked my doctor for him to be tested for ADD/HD, but she doesnt seem to think he has that, I cant even take him in her office cause he doent listin, and he is bouncing of the doctors bed, and playing with the equipment, and I ask him to sit down and he wont listin, so Like should i be asking for another doctor, like she see's that im stressed out and my son wont listin, and she tells me to be consistant, and I have tried my best with everything, now in this past week, he was playing outside, and pulled a little girls pants down, and sprayed a growling cat down with a hose, after we had just came back from a family trip, like Im lost here, I dont know if Ive lost as a mother, or if im just not getting the help i need, letters get sent home saying he doesnt listin, like im lost here, any advice PLEASE..

I am not with his father, we broke up shortly after i convieved my son, He does call my daughter's father dad, as he has been with us for 4 yrs, my son does listin to others and will say he knows what he does is wrong, but then he does something else to get in trouble, Ive been through some childcare ladys who have quit cause they will tell my son he cant do something, and he will say you cant tell me what to do, but the saddest thing is that he can be so sensitve, and when we have a good day, he notices it, and says, see mummy, we can have a good day, when everyone gets along, and i try to always praise him for the good that he does.

Has anyone been able to control him?  Does he have a dad in the picture?  Are there others - some teachers,  coaches,  uncles,  etc., who are able to get him to behave?
