4 year old with severe chronic constipation

My child is 4 1/2 years old and has had chronic constipation for some time now. As an infant, we were able to use corn syrup in his formula to get his bowels to move but now as he has gotten older, he is going for some time with out BMs. He has been on Miralax before and it did the job but now we are at a point that even it isn't working. When he does have BMs they are hard and very large. His last bowel movement actually tore his anus and he was bleeding. That was a week and a half ago, and we still have not seen another BM since. He has been soiling his underwear so we know he has to go. We have given him laxatives, enemas, and two days ago, we took him to the pediatrician for help. She confirmed through physical exam that he was severely backed up and gave us Kondremul 50% to get him cleaned out. He has had 3 doses and still we have not seen a BM. We need help. I don't understand and I want my child to feel better and have confidence in preschool that he will not have accidents...

What is his diet like. You should look at that. No sugar, lots of fruit and veg. Lots of water, lots of excersise. Set a time for siting to try and have a bowel movement. There are most likely lots of reasons for this general make up diet, fear of BM or getting in trouble. If you ignore something long enough you get good at ignoring it. Don:t take it personally just work the solutions, do your best to not get upset, don:t ask inform it is time to use the restroom, sorry you do not like the food but thats what we are having. When was the last time you saw the skeliton of a child when there was food available.
