Masturbating is not related to acne. Ask your doctor about this because he/sge may give you a prescritpion medication to help. There's no reason to be alarmed about remaining fertile (being able to have children). The reason for the enlargement may be minor and it could resolve on its own. But it's risky to take that option, because it's important for the doctor to figure out what is causing the enlargement. You needn't mention anything to your parents about masturbation - that is beside the point. Just let them know about the swelling in your testicle and that you want to see the doctor. I assume they will not only understand, but agree with you that it is important to visit the doctor. Talk with the parent in whom you have the most comfort.

An enlarged testicle can be caused by a number of conditions, including (but not limited to) a cyst, infection, inflammation. It is a condition that should be evalauted, so ask your parents to make an appointment with your doctor. It is true that enlargement of the testicle can be caused by excessive force during masturbation, but there's no reason to assume that is the cause. In the future, when you masturbate, try not to push on the testicles themselves. Now be sure to mention this to your parents - it shouldn't be ignored.
