Toilet trained but peeing & pooping in pants

My boy has just turned 3 y/o and has been toilet trained for the past 6 months.  Recently he has given up his afternoon sleep & when he is tired or when he is having a tantrum, he refused to go to the toilet (even with me - as we used to pee together - which encouraged him to stand & aim in the right direction).
His mum has also begun to do some work from home so there is a little less direct attention.  He has a little brother, but we begun the training after he come home from the hospital.
It all worked so well for the first 6 months, even to the point of taking himself by himself.  We dont punish "accidents" - but Im not sure I want to go through the - clean yourself up & your undies - method - as this seems to be punitive.
What should we do to redirect him backl to the toilet?

Thanks for your thoughts - & coincidentally this is what we have done.  We have gone back to "sticker" rewards whenever he does the right thing & straight away on the first day - no accidents and good (almost) communnication about his toileting.

I recently I had a similiar problem with my two year old daughter. She learned really fast and can climb onto the big pottie all by herself, although wiping is still an issue. When we began training her to go pottie, I think she enjoyed the accolades and congratulations that accompanied her success. Now, with her frequent success I believe we have come to expect this from her and have shown less appreciation for her efforts and in turn she has put less effort into it. There are times where playing now takes precedence over going pottie and she will hold it until she wets herself. i think its just a matter of reinforcing the good deeds and the importance of going pottie. Try not to forget to show appreciation for when the child does have success.
