2yr old developmentally slow?

I have a 2 yr old step son who I am very concerned about.  He does not seem like he is where he should be.  He is not speaking more than one word at a time.  When you talk to him or ask him a question he repeats the last word that you say. Sometimes you can get him to say love you but only after you have said it for him to repeat it.  If you ask him a yes or no question he is unable to answer it he makes a winy noise so I correct him and say that he needs to use his words yes or no and he repeats no... He still has a soft spot on top of his head.  he is unable to fully understand how to use a fork or spoon,  his stomach sticks out like a one of the children you see in Africa (malnourished) yet he eat plenty.. Also one of his feet is turned inward so he walks with a gimp...  I am not sure what to do we only have him 2 days a week... his mother is still feeding him formula (soy) because she thinks he is lactose intolerant yet she feeds him cheese and ice cream.. She is still feeding him a bottle at night... he does not get one at our house.  He is terrified of loud noises such as the vacuum cleaner...  when he comes over he just sits in one spot and does not move for hours when you try to get him to play he has a hard time comprehending game like stick the square in the square hole.  I guess my question is am I reading into this to much should I be as concerned as I am... if so what could possible be going on?? I would take him to the Dr. however his mother has prevented it every time we have made an appointment. Something just does not seem right about him.. and everyone out side his family thinks the same when they meet him.

He is not pigeon toed his hips are rotated inward.. and as for the Dr. appoinments it is his father that keeps trying to make them and everytime he does she pulls visitation.  She is not giving him soy milk she is giving him soy formula which according to dr's after 12mo's of age the can get iron posioning because of the amount of iron they get ing their normal diet in addition to formula.  I have been in the picture since before he was born.. this was a girl that My husband had a one night stand with before we met and she decided to keep the baby and he wants to do the right thing and be in his life as much as possible.. I have one child from a previous marrage and one with my current husband, I understand that every child develops differently but , myself and my husband can tell that the 2 yr old is behind.

My kids are 2 and 4 and they drink soy milk there is nothing wrong with that. In this case you need to let the father handle it. My youngest one is pigeon toed also the doctors will tell you to wait it will correct itself, and his stomach also pooches out. How long have you been in the picture. This is just my opinion but you dont have the right to make him a doctors appt, your husband does but not you and I can see why the mother would prevent it if you made the appt and was trying to take him. You only see him 2 days a week. If your that concerned you need to tell the father and let him talk to the mother.
