His weight below the 5th percentile for his age. Are you giving him nutrient dense drinks? For underweight children, they should consume daily all the nutrients with protein foods for all meals and snacks. Suggest giving him Pediasure in place of milk or make your own milkshakes from milk or yogurt (milk, ice cream, & fruit blended; or yogurt, ice cream, & fruit blended). He should have a protein source each meal --- meat / fish or cheese or yogurt; with a carbohydrate --- bread, pasta, rice, cereal; and a fat ---- pat of butter or mayonnaise if you made a sandwich (children under 3 years old need foods with regular fat for brain development). You stated he is becoming a picky eater; hopefully he will still eat some vegetables and fruit at each meal too. Suggest giving him a daily children chewable multi-vitamin. The most important thing is to RELAX. You do not want mealtimes to be a battle of the wits between your child and you. Remember meals need to be joyous for you and him. Slowly introduce new foods, and if he does not want it, do not make a fuss over it. Let it go and enjoy the meal. Try again in a few months. Sometimes it takes 5-9 times to introduce a food to a child that they will accept the new food. Hoped this helped you.

Blend the fruit in the milkshakes, such as bananas and some avocado. He will not taste the avocado with the banana flavor. You can try strawberries or other fruits, too.
