Very High Triglycerides, please help.

I am 40 years old from Nepal. My lipid profile reading is as follows:
Cholesterol  238.5
Triglycerides 466 mg/dL
HDL    34.7
LDL     112.60

My blood pressure is 120/70. With the above reading, do I need to urgently start medication? Or I can lower my Triglycerides with exercises and weight loss?

Please suggest, Doctor suggest me to take Fenolip 160mg.
