vomits clear fluid early am

tony i have a * yr old shitzu that was diagnosed with crf in oct 2015 . she is taking benazapril and amlodipine bid and plavix once at nite her last  she also gets ranitidine every 12 hours her latest bloodwork had her bun at 48 and creatine at 2.5 , we have her on a senior diet with low protien .still trying to get her to eat a kd diet, i feed her 3-4 times a day i give her chopped green cabbage with meals and baked sweet potatoe for fiber and bulk, only treats she gets is small portion of baby carrots, she has vomited a clear liquid smells like bile, a small volume 3-4 times this last week it is always early in the am 4;00am to 8;00am , her last intake of any food is around 9;00pm, otherwise she eats good acts normal no diarrhea drinks water no dehydration issues, anything i can do to help her with this ?

A few questions before replying properly ...

Was the Benazapril prescribed for high blood pressure and, if so, is the BP now controlled? Or was it prescribed to improve blood flow and volume to help the kidney disease?

Plavix is an unusual medication in dogs. Are you aware of why your vet prescribed this? If not, please ask. If it is for glomerular disease, then it is not the best medication. Studies suggest plain aspirin is actually slightly safer.

Amlodipine is a calcium channel blocker and typically given for blood pressure or another circulatory condition. It is also used to assist kidney failure, by improving blood flow. However, it isn't the first choice or recommeded first choice in kidney failure for an otherwise healthy dog.

The diet sounds fine, but feeding the daily volume of food portioned into 6 or 8 smaller meals given every 2-3 hours would certainly help the kidneys deal with it. It may also reduce nausea and bile production. These smaller portions remove much f the stress placed on kidneys by food intake - although I realise it isn't always practical to feed in this way. And if your dog is small for the breed, then tiny portions could mean one or two desertspoons at most.

Creatinine at 2.5 puts her into stage 3 (just) of kidney failure, assuming other levels confirm this. Lower protein is certainly therefore appropriate, but importantly, high-quality rather than low-quality protein is just as important. That means either a good quality kd dog food (canned) or human grade meats and poultry.

Even more important is the phosphorus content of food. Phosphorus must be severely restricted, so make yourself aware of what is in the foods you are feeding. You didn't mention phosphorus in the bloodwork - has this test been done? If so, what is the reading?

tony thanks for your response the benazepril is to treat the hypertension and to reduce protein loss through the kidneys the amlodipine also for hypertension and control her plood pressure which was elevated the plavix is to reduce the chance of blood clots i also give her fish oil and give her chopped green cabbage with meals , i have started to wake up and give her a meal at 2;00 am and seems to have stopped the vomiting of bile . when karly was diagnosed in oct of 2015 she weighed 9.50 pds she has lost 2 to 3 tenths of a pound every couple of months and she now weighs 7.38 pounds her vet is pleased with her weight , i think she is too skinny.she now eats her hills k d food good and likes it . hope that keeps up her bun and creatine have only changed a tiny amount since oct so i am encouraged with that, she was having a few accidents at nite about 4 weeks ago and the vet increases the benazepril a quarter of a tablet at the evening dose it was 2.5 mg every 12 hours before that , could the extra benazepril have contributed to the vomiting ? karly was spilling a little more protien in her urine so the increase was for that .will take her back in 4 weeks for new labs . again thanks for your help !

Hi. I think everything is being done appropriately right now, so just keep doing what you and your vet are doing.

As phosphorus is within good range, your vet probably thinks a phosphate binder is not yet needed. However, current research tells us that creatinine increases and the likelihood of uraemia means a lack of control of serum phosphorus is having adverse effects on PTH (parathyroid hormone). Diet restriction alone will not prevent this domino effect of symptoms. I would suggest starting a phosphate binder now (aluminium hydroxide being my preference).

Benazepril can cause vomiting as a side effect, but I think it is more likely attributed to a build up of toxins in the gastric tract. This is typical of mid to late stage kidney failure.
