PFT Results COPD

I'm 41 and smoked roughly 20 pack years. I've had breathing issues for years due to anxiety and had plenty of clear chest X-rays, mostly done due to panic attacks . Last September I went to the ER due to heart palps and shortness of breath, the ER doc said lungs clear,was anxiety related. During my routine GP visit he stated the X-Ray done in the ER showed hyperinflated lungs and referred me to a pulmonologist, he did a CT which showed moderate emphysema which is primarily paraseptal according to the report. During my PFT I did not have to do the bronchodilator/inhaler. My results were FVC Pred 4.68 Pre 4.72 %Pred 101 , FEV1 Pred 3.62 Pre 3.73 %Pred 103, FEV1/FVC Pred 77.35 Pre 79.05 %Pred 102.  Comments at the bottom states good patient effort, post test not done, FVC and FEV1 80% or greater. My pulmonologist basically said you have emphysema, stop smoking and I'll see you in 6 months. I asked what stage and he did not say, I asked about my PFT and he just said I'm happy with that? Any imput would be helpful.

i agree, CT doesn't match PFT. Looked at my CT results again and here is the word for word results. No adenopathy,heart is normal size. Moderate emphysema which is primarily paraseptal. There is apical pleural scarring bilateral. There is mild bronchial wall thickening which may reflect bronchitis or COPD. No consolidation, pleural effusion or suspicious lung nodule.  I don't even thing my pulmonologist even looked at the actual scans, just based his diagnosis primarily on the radiologist interpretation and the fact I'm a smoker.

You did not have to do the bronchodilator because you have normal PFT's.

DLCO is good, you have nothing to worry about. Stop smoking if you haven't already. I can't see how the Pulmo is getting anything other than normal from the PFT results. The CT needs an explanation.

my doctor is going by the (79.05) on my FEV1/FVC as said I don't understand how to read the spirometry results? On this test it shows Pred:77.35 Pre:79.05 and %Pred 102 . Not sure if my number is 79.05 which would put me at mild/moderate or the 102?

Yes my DLCO was Pred 29.60, Pre 26.17. % Pred 88 . I'm still confused on the results or rather the numbers in general

just stop. I told a doctor proudly I'd stopped smoking at age 70, i'm now 86. He said: Well, you smoked enough didn't you'.  So don't expect a pat on the back

With those PFT results however it could be mild. Stay well. Could be worse and have bad PFTS and the same scan result.

Amazing, emphysema with very normal PFT values. It happens, google Lone Emphysema. Have you had your DLCO checked.
