elevated gastrin level

Hello again,

I wrote a while ago about my anaemia and allergy to iron infusion.

I have had some further tests done.  A lot of the results seem abnormal and I was wondering if you can interpret them for me.  I didn't get to see my doctor last time I went to hospital for my routine transfusion.  I'm booked for my 3rd yearly gastroscopy/ colonoscopy in a months time.

Hb          72      L
RCC         0.24  L
WCC        2.2    L
ANC         1.3    L
Lymph      0.4    L
ESR        40      H

B2 M          3.73 H

Urea           8.3    H
eGFR        56       L
Chol            3.0    L
Fe less than 5      L

Soluble transferrin receptor     3.75    H
Gastrin        202    H

Fe  less than            5  L
Saturation less than  7  L
Ferritin                     7  L
B12                       112 L
Active B12               26 L

ANA         positive speckled  1:40
P-ANCA    positive atypical

I know that some of the values might be different in different countries, that's why I put the L or H behind the result.

All other tests were within normal limits.

I have been sick for 5 years with no definite diagnosis despite a lot of tests, my doctors just treat the signs/symptoms.

Thank you for your time.

Hi,  yes I have had a number of bone marrow biopsies.  All but one showed no iron stores.  One showed normal iron stores but still showed dyserythropoiesis with dehaemaglobinisation.

I spoke to the haematologist today and he says I have a definite malabsorption problem.  He also thinks I have small bowel telangectasia which is contributing to the problem.  I am going for a pill cam and repeat attempt at iron infusion (in ICU, in case of anaphylaxis/steven johnsons syndrome....which I have had before).  I have started B12 injections and will have these weekly for a while.  He has also started me on B6 and folic acid.  He also told me that I have to be compliant with the PPI that I was prescribed a few years ago and which I have been refusing to take.

I was found to have candidiasis of the oesophagus on gastroscopy and that is being treated with fluconazole. I was also found to have diffuse oesophagitis, Barretts oesophagus, mild chronic gastritis with multiple small erosions.  Duodenum normal.  Three hyperplastic adenomatous polyps removed from sigmoid colon, otherwise normal colonoscopy.

I asked for your input because, I was not confident in my doctor.  But I have a new doctor and I feel much more confident and feel as if I can trust his judgement.

Thank you for you response.

Was bone marrow biopsy or bone marrow aspiration test done?
