Heart Problems

I recently ended up in Emergency room with breathing problems and heart beating strangely. I had teh following test performed and these are the results. What are these telling me?

ECG   Sinus rhythm with premature atrial complexes  ST & T wave abnormality, consider anterolateral ischemia, abnormal ECG  

X-Ray of Chest  Lungs are markedly hyperinflated. Emphysema.

Cat Scan of Chest with IV Contrast  There is ulcerated plaque in posterior descending thoracic aorta near the diaphragmatic hiatus. Also note is made of coronary arterial calcification.

There is diffuse changes of CODP throughout both lungs, predominantly at the lung apices. There are scattered calcified granulomata with calcified left paratracheal and AP window lymph nodes from prior granulomatous infection.  In the extreme right lung apex, there is a 5-6 mm spiculated nodular density anteriorly. The may represent a very early neoplasm or scar.

IMPRESSION: Coronary artery calcification, COPD, very small 5-6 mm spiculated nodular right lung apex. Not clearly present on prior exam although small and could represent a scar or a very early neoplasm. Need follow up CT in 4-6 months.

Echocardiogram  Conclusions: The left ventricular chamber size is severely dilated, there is global hypokinesis of left ventricle with minor regional variation, there is severely decreased left ventricular systolic function, estimated ejection fraction is 30-35%, Mild to moderate aortic regurgitation is present, there is mild aortic stenosis, the aortic valve gradient mean is 14.5 mmHg.

MRI of Brain  FINDINGS

Something strange show up in this that may be heart related?

The ventricles and sulci are within normal limits. There is no midline shift or mass effect. No intracranial hemorrhage or pathologic enhancement is seen. There are punctate areas of increased T2 signal within the periventricular and subcortical white matter most likely related to chronic microvascular ischemic disease. No restricted effusion is seen.

Can any one give me some insight in to these conditions?

Talking about your heart rhythm not being normal and a possible heart attack or heart damage causing this.  There is calcification in the coronary artery and narrowing of the aortic valve.  Left ventricle has severe decreased function which results in the low EF.  You have Chronic Obstructive Pulmonary Disease along with Congestive Heart Failure with calcification in arteries and narrowing of the aortic valve along with emphysema.  How are your kidneys functioning?  If you are having problems breathing and it is caused by the aortic valve narrowing, the surgeons can do a cardiac catherization which means that they would go mostly likely into the neck to the aortic valve and open it up a certain percentage.  Do you have any diabetes?  You will have to monitor the aortic stenosis and when it becomes moderate you may want to talk to your doctors about the catherization.  I have seen this valve in family members go from mild to severe in a matter of 3 years, so make sure that you are getting it checked often.  Make sure to keep all your doctor appointments and take your medication as prescribed by your doctor.  Stains like Lipitor help reduce cholesterol.  Make sure that your blood pressure is staying down and you are keeping water off.  If you gain a pound a day, you will need to give your doctor a call.  This would be water building up and you do not want to have any water buildup.  Talk to your doctor about diet if you are not already on a strict diet.  Take care.

Thanks for the information.

My one kidney looks like it is doing ok at this time, other one removed in 1999 Renal Cell Cancer (RCC).

weight 215 down from 245 a month ago. I am a diabetic and control it with a modified Diabetic diet.

Will see my heart doctor on Wednesday. I plan on following what thes edoctors have to say, the feeling of not getting enough air is a feeling I do not llike, it i slike drowning!
