Hi,
Having a small piece of tooth or bone left over from an extraction is normal.  It does happen.
They have a tendency to work their way to the surface and come out.

They swallowing problem may still be from residual anesthetic.  Try rinsing your mouth with warm salt water (1/4 tsp per 8oz of warm water) a few times a day to help with healing.

Best to you.
