I had simalar trouble with unwanted thoughts. I think it was the result of bullying I had and other troubles. If you can recover from any trouble u have had and try and get a remedy for any trouble in your life you should start to heal.
I had to ask a Church minister to pray with me and that helped.
I also asked for Holy Water from the Catholic Church. You put a drop on your forehead and then do the sign of the cross over your heart [in the name of the Father , the Son and the Holy Spirit] and ask to be blessed. Most of my troubles are gone since then. I have some faith in Jesus.

I've been procrastinating about answering this post.
I think maybe because there is stuff that you don't want to look at in your own life.  When the stress, etc builds I think the thoughts tend to become more negative/ more violent.  I could be wrong.
Medication could possibly help you manage them better.  It may or may not cause them to get worse.
I would possibly encourage you to see a therapist to help you work out what is going on.
I have no doubt though that the doctor will be able to sum it all up in about one sentence.
I hope you get your answer.

these bad thoughts are  generated by your brain, trying to get your attention and get you to think about the problems in living you have that you are not facing.  Facing them is the way to eliminate this brain mechanism. You should either see a therapist or go to www.myvirtualshrink.com for self therapy.
