BURNING ROOTS OF HAIR

A 60 year old man with diabetes, uses insulin, and has never shaved his beard. Lately, he has been suffering greatly from a burning sensation coming from the roots of his beard. The roots have a swollen substance on the base, as can be seen in the picture here. So much so, that he has to pull out his hair to ease the pain. We have not been able to find out what this is.
