Hard scar-like lines under my foreskin, inflammation and pain please help?

about 5 months ago i noticed that the skin from my penis head started to dry almost to the sheding part.
I thought it's some kind of yeast infection so i took canesten cream used it for 4 weeks, nothing my skin was fine but if i stopped using the cream it went back to super dry skin.

After about 2 months and tons of different antifungal creams it developed from rash to 2 small plagues on the sides of my penis under the gland and after that nasty discharge and super swelling and pain.
I went to the doctor and the dermatologist took a sample from the fluid and sent it to the lab, it was e-coli.He said i most probably took it from sex.

It prescribed Amikacin IM for 7 days. Things were improving, the discharge went away, the skin started healing BUT not entierly. The parts of my penis where i had the plagues before were still swallen and if you looked closer you could see some small yellow dot like bumps that felt hard on touch. Like i had some sand grains right under my skin.

I went back to the dermatologist, he said i should take 3 more days of Amikacin just to be sure and after 2 weeks test my urine. I did that and it came clear, no germs no nothing but the swelling and the yellow dots didn't dissapear also the area under my foreskin (where the infection was) was very sensible to touch and from time to time i had burning sensations that spread all the way to my testicles(also my scrotum was red in color something unusual i never had that before the infection.)

I went back again to the dermatologist and he said (after a 5 minutes consult .. ) that the sewlling the burning sensations and the sensibility are normal and that i should give it time to heal at least 1 month, also he prescribed Aciclovir for 7 days (pills + cream) because he said i might have a small case of herpes too.
I took that , nothing no change.. I thought i should wait and see if the situation improves like the doctor said but it didn't; things very slowly started to deteriorate at first some scarring appeared in the form of thin lines that you could feel at touch around the parts that were infected before (they were paralel to my penis)

Now my penis is slightly more swallen than before, i sometimes have blunt pain through the hole penis body small to medium in intensity, burning sensations and the sensibility of the skin under my foreskin is even greater; small skin lesions appeared in the 2 places where i had the infection before they heal and reappear (or they never heal completely
the hard scar like lines are longer also when i wash i noticed that there are discolorations on my gland ( red stains ) that dissapear somewhat after a while but some of them are still visible.  

Pls help me if you can, i'm not sure if i should go to that dermatologist again because all he's going is making my wallet dry with no results.
I can't go to another medic atm because there is no other medic in my small town and i need way more money to go in a city at a decent hospital.

Can you give the dermatologist that you saw a call and discuss the issue via phone?  If you do go back, you want to make sure they understand why you are going back.  If you are too worried that they won't do anything, it does sound like your best option is to see another doctor.  Are you concerned about a lack of insurance or is it the travel time to get there?  The doctors will usually work with you on payment, especially if you don't have insurance- so call around and see what you can do before you are sure that you won't have the money right now.
