Intense Itchy Swelling Rapid Spreading Rash

First... I am FED UP.
Second if this seems a little long PLEASE do not turn away. My Doctors and Dermatologist, ER, Vets, Pre-Vet students, family and friends anyone I have asked, have no idea what I have.

I have this rash which covers most of my skin from head to toe. It is extremely itchy. And swells like i have silicone injections under my skin. This has been Spreading from the size of in ingrown hair on my hip to 10" patches on my thighs, shins, feet, butt, sides of my torso, inside of my belly button, my armpits, triceps, elbows, and underside of my forearms. But is does not seem to really effect the top side of my fore arm or biceps. My face is not effected by this. My scalp has also shown up with these "boggy" dime sized itchy swells.

Timeline:
     It started out in Feb. What looked liked an ingrown hair on my hip. Thinking this i left it alone. By about May it still has not fully healed, but now another pimple (no pus) like bump appeared. This is now becoming itchy. Ive have had worse on my body so I still didnt stress over it. Two weeks later it is now very itchy  and now to appear as if it is RingWorm. I had this as a child a couple times and used over the counter meds. No effect.
     This is when I start to become worried, it being so close to my groin. I went to my Doc. First words she said after she saw it was "ummm... wow." Her face said everything. She had no idea what it was. Sent me to a Dermatologist. And once again i get "oh boy... what is this we have here." I got Steroid injections in the boggy spots, a Biopsy, and some fungal creams and shampoo. Two weeks later, no change, but biopsy results are in. "Well we now know its not fungi" , so what is it doc, "we still do not know."  
   June, by now the rash has spread over my legs, belly button, and now starting on my arms. Every time i scratched the bumps it became VERY swollen, almost like how i react with mosquito bites. If i dont touch it, its not very swollen, but if i do, it swells to about the size of a quarter.
   After searching all over the internet and asking around, I went back to my doctor and asked if its possible that it can be Scabies. She gave me Permethrin. I put it on head to toe and kept it on for 12 hrs. Incredibly next few days the itch lessened and the bumps started to leave. Some scaring around my hips, but who cares its the bumps and insane bloody mess itching has gone.
   OK, im Cured!

   Now it is October. And now within a matter of 3 days the bumps returned, and now worse then ever. Now instead of  a bunch of separate bumps I have a solid Patches now which are red, Swollen, incredibly itchy. Its on my chest hands everywhere... except my face, penis, and scrotum.

So now what?

Ive tried:
Selenium sulfide lotin USP 2.5% for my hair
Triamicinolone .1% for "contact dermatitis"
Clotrimazole 1% for fungi.
Permethrin 5% about 3 times now for "scabies"

Side notes:
1.Almost every bump starts out as a single bump. But strangely it then splits into about 5 smaller bumps in the shape of a pentagon. Until they split again and start to spread out in a ring formation. Then when it is in close proximity to other bumps in turns into a swell where the individual bumps are not visible anymore.
2. If I keep close on, the itch really subsides. But once i remove my shirt or even roll up my sleeve the itch comes back with an additude.

My history:
I am a 23 yr old male, have been in good condition my entire life (besides the fact that I have had my ACL replaced 2 yrs ago, if it makes a difference.) I have had seasonal allergies for about 8 yrs ( itchy eyes, runny nose) nothing a few zyrtec couldnt control. I do get an itchy throat if i eat the skin of apples, pears, or plumbs.

I should probably be going to the Doctors as much as possible. But i cannot take off any more time from work. And finding Doctors that are available when i am has proven difficult. So now I ask on this Forum for some help, advice, opinion, anything.

I am open to any question what-so-ever about my personal health history, drug usage, job etc. what ever it takes to help get an answer

I thank everyone in advance. If you have taken the time to read this, please, anything will help now.

I will try to attach some pics. I believe they are right before i tried the Permethrin the first time, which looks like nothing compared to what i have now. i will update when i get back to my comp.

Hello!

Annuloma Granulare seems to be a combination of wheat and Gluten allergi. I have had this disease for some time and when i stop eating gluten
and Wheat it dissapears/dyes out after 3.-4 weeks.

If the onset coincides with going vegan you might have your answer right there.  I am all for clean food and healthy unprocessed eating BUT you may have some inate body requirement that needs meat protien.  Try some free range grass fed beef to see if by very slowly adding this to your diet maybe something will balance back out.
I have intermittent allergic reactions that I think are the symptom of a deeper issue.  For 3 years I was allergic to MSG with violent body covering hive outbreaks caused by the consumption of the tiniest amount of the compound.  Then that passed and a few years ago I had to quit coffee because it made my face break out with Rosacea.  Now that is over but beer makes a scaly patch on my back flare up and I get full body itches with no rash.  I have had Granular Anuloma for years and nothing clears it up.
I would bet on something in the environment, mold maybe or food.
CHeck into a local naturopathic college, Here in Seattle we have Bastyr University and they can do a top notch job of see beyond the normal boundries of western medicine.

How bout a possible allergy to something unusual like dust mites. If they are in your bedding or mattress and multiplying, the problem would gradually get worse. Pillow would protect your face, underwear would protect genitals from contact. I would recommend you see an allergist. Go to this web site.  http://www.healthology.com/allergies/article36.htm   It gives a good overview of what an allergist will do to diagnose a possible allergy and help you find a good allergist in your area. I have realized how clueless they are. I understand that there can be many diff problems i can have. But all they wanna do is try different meds to see if it works or not. Which doesnt make sense to me, being how fast it is progressing. 9 months now, and my body being 80% covered and they all take it as lightly as if it is a simple pimple.

This started about the time I became Vegan. It was an overnight decision. Completely stopped All animal products from food to clothes. Within the last 3 or 4 months i have also stopped eating Canned food. Even trying to come as close as i can to not eat machine processed foods.

So all i pretty much eat now are my fresh veggies, fruits and nuts, rice, and pastas.

I live in NYC, NY. Does anyone know what else might be in this water? or what contaminates can cause such a problem.

I dont know if it is possible in my case, but what im scared of is if this rash covers me, and it is due to a Contact dermatitis or allergic reaction, can i go into Anaphylactic shock? This would be bad being i work with Power tools and Heavy machines.

Thanks willy. Out side the box? lol Ive been lookin outside, ontop, bottom, side, even making up my own box.

Detox? how do i detox? you mean one of those body cleansing drinks?

The derm should recognize mumps or hives. But most of them are clueless. Did you detox and change your diet. Any new products like laundry detergents, dryer pads, soap ,shampoo, vitamins, herbal tea, food etc.
I know someone who had a reaction to well water. The water had fertilizer in it from a local farm. He had a shallow well and had to get lawyers to get the problem fixed.
If you live in a city they use chlorine, you could be allergic to that.
Think outside the box! The doctors won't do there job.

Also was just thinking... I have been having back discomfort. The only way to relieve it is to crack it. Being that I have always been flexible, i have always been able to crack almost every joint in my body. I have read something about Lower back pain problems and them causing rashes. Just a thought.
