Doc, it reappeared again for the 3rd time :(
Basically when it appeared the first time, i applied the cream i mentioned above for 5 days and it disappeared. 2 days later, it reappeared
Then i applied the cream again and 5 days later it disappeared again. This time I kept applying the cream for another 4 days (so total 9 days) and yes I never had any more of these red rashes during those 4 days...but then I stopped applying the cream and 2 days later it reappeared again!
I then bought a OTC antifungal cream and there I can read their instruction:
"You have to keep applying the cream for minimum a week after all symptoms gone"
So basically i need to apply the cream for at least 1-2 weeks after all symptoms (red rashes) gone right, instead of stop using it? Problem is, im afraid the fungal will be resistent to my cream considering the OTC cream has the same ingredient (miconazole nitrate). I have applied the new cream 2 days so far and nothing has improved...
do i need to change cream to another antifungal cream that's using different ingredient?

As of now, my current redness has subsided since yesterday but i still applied the mixed cream that i mentioned earlier and plan to continue using it until tomorrow. I have been to my dermatologist again and he said no need to continue using it, as long as it's been subsided, then it's good...
I didnt get any oral antifungal treatment, i was only prescribed lameson 16 gram, which indication is for allergic and inflammatory condition (and not fungal treatment). Do you think by just using this oral drug and the previous antifungal cream for the next few days it would kill the fungi and it won't come back?

Fungal infections are quite common and present as red to pink patches with scaling at times in the groin area. These are itchy and can spread to nearby areas quite fast. Also fungal infections subside on applying fungal creams as well as steroid creams- in your case you applied a mixture of both. In order to get a complete relief, you should be prescribed a course of oral antifungals along with a topical antifungal.
Scaling of fungal infections can get attached to the undergarments but a thorough washing takes off the fungal scales and there is no need for you to dispose those undergarments.
There isn't any need to panic as fungal infections are easily treatable are not serious though the itching can be frustrating at times.
Just discuss with your dermatologist to start a course of oral antifungals for you if the problem persists.

an update,
today i visited the same dermatologist who prescribed me mixed cream of benoson M and fuladic, and he confirmed it's dermatitis on my scrotum and yes it has fungal infection, that's why the previous cream worked before it reappeared.
However, i totally forgot to ask him if this fungi could lived in the underwear and reinfected me?
i mean, i had a thought that the fungi might "jump" from my scrotum to my underwear when i had active infection last week and then i reuse that same underwear 1-2 days ago so it reinfected me, could it be possible? if yes, do i have to just throw away all my underwears and buy new ones?
