I also have roseacea a moderate case They prescribed metrocreme did not work I also cannot afford to go to doctors so I researched and came up with my own medicine Use cetaphil bar soap only for dry sensitive skin next drink at least 4 qts water a day very important also no caffeine whatsoever you can switch to anything decaffinated which includes chocolate no diet sodas no spicy foods eat foods and drink liquids at room temperature when you feel face flushing drink cold water my face is photosensetive wear sunscreen always as even indoor lighting bothers my face I found a natural alternitive to help my face which I just started using so far it tamed it down a little will let you know if it does what it says remember you never get rid of it you just manage it Good Luck Anne

To be honest, I don't know of any over-the-counter treatments or skin care routines which make much difference to rosacea, one way or the other (except of course for avoiding irritants, such as harsh soaps, exfoliants, or possibly benzoyl peroxide.)
Perhaps your friend can get someone to give him a prescription for a cream containing 0.75% or 1% metronidazole, to be applied once or twice a day for 6 weeks. No side-effects associated with that. If it works, he can use it intermittently. If not, he should probably see a dermatologist for more precise diagnosis, and perhpas oral treatment.
