Soft Fibroma/Fibroma Molle in the penis

Dear Doctor,

I am 33 years old and in the last 8 years I suffer with a problem in my penis.
After consulting more than 5 doctors (general practitioner, dermatologist and urologist) giving me the more different diagnostics I did a biopsy a year ago that concluded I have SOFT FIBROMA (FIBROMA MOLLE) - [See the biopsy in the pictures].

In the beginning, a kind of white granules showed up inside my foreskin, but in the last 5 years it spread in my glans, mainly in the front area.
In the glans and in its edge the problem become in a sort of a "white dead skin" that can easily be removed by nail. There's no pain, just a slight itch sometimes and, once in a while, I feel my penis sore.
In the last 7 months, for 3 times a clot appeared in the extremity of the meatus, becoming in the "white dead skin".

I was relieved that it is not a STD, but physicians don't know the source of the problem. I was wondering, maybe it could be emotional, because I always had blocks and a lot of anxiety in sexual matters.
Moreover, symptoms usually remain stable, but it get worse in a certain moments that normally endure for a couple of days returning to its previous state. However, in general it getting worse.
Nevertheless, I couldn't relate this "crisis period" to any external factor, neither emotional nor environmental.

I would like to know, please, if there is some kind of medication or treatment that can resolve my problem. If there is nothing, I would like to know if there is something I can do to improve the aesthetic aspect, as a peeling for example.
The Dermatologist suggested a surgical scraping in the glans to remove the dead skin, but I think this is very aggressive and probably will leave scars, without solving the problem definitively.

I will really appreciate your help.

Thank you in advance.

Frank

Dear Frank,

Welcome to the Dermatology Expert Forum!

Fibroma molle (skin tags) can be removed by surgical scissors, electrosurgery, or cryosurgery.

In general, clinical findings and biopsy results should be correlated before final diagnosis is established.

In your case, based on description of lesions and evolution of the process that you provided, several other diseases should be considered as differential diagnoses including Lichen nitidus, Lichen planus, Lichen sclerosus, Seborrhoic keratosis, Porokeratosis. Therefore, your dermatologist might recommend another biopsy.

Bottom line, all of these disorders are treatable, and should improve/subside with proper dermatological therapy.

Wishing You Optimal Health,

A rectification:

Only some parts (the worse) can be removed with nail. The most of it seems to be subcutaneous.

Thank you.
