I am 22 year boy.
from the past one month i am feeling that m penis foreskin is becoming narrow and tight , also i feel difficulty in pulling down my foreskin.
recently.. my foreskin tip has become tight with a particular circumference.
i don't have problem while urinating and there is no itching in my penis neither there is pus etc...
I have consulted to dermatologist. He wrote me VRDL test and sugar test , all resulted in negative. After that he advised me to wash it with saline water and apply SOFRADEX F cream on my penis on and underneath of foreskin for 1 week, earlier it showed a little improvement and in 2nd week the problem was as it is. Hence he gave me tablet FABIZOL 150mg, but there was no improvement.. Further my under underneath of foreskin gets red and patchy as if it got bruised but there was no pain. Hence i showed it to another dermatologist he referred the diseases as BALANTIS XEROTRIA (something like that) and advised me to discontinue earlier medicine and Rx me OINTMENT TENOVATE GN and tablet MICRODOX DT.  BUT applying this ointment underneath and on the foreskin I found that my red skin got worse and stared paining and give painfull sensation whenever I use to wash it with water... now when my inner skin is in bad red condition and the dermatologist has advised me to discontinue the ointment and has now gave Rx of Tablet NIZRAL 200 and capsule NOVACLOX LB... But still my underneath of foreskin is very red and sensitive.. and today morning when I was cleaning with water my underneath skin was peeling out and a thin layer of skin peeled out off my penis head...now its about 25th day i have consulted the dermatologist but my problem is getting worse day by day... please help me and my tightness of foreskin is still there ... no improvement in that...

Apart from balanitis skin which we had discussed in your earlier query, phimosis penis can also present with such symptoms. Phimosis is a constriction of the opening of the foreskin so that it cannot be drawn back over the tip of the penis. Pathological causes of non-retraction of prepuce can arise from inflammation of glans penis or balanitis. Treatment would include steroid cream application and balloon dilatation. Surgical treatment includes circumcision.

If the symptoms persist then get an evaluation done from a dermatologist.
It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. Of course, circumsicion is the last step. To me, I tried everything and nothing helped me. Suddenly red spots showed in my glans and my foreskin was swollen,. no itching, no pain. Got tested for STDs several times with negative results. Doctors prescribed me antibiotics, fluconazole for yeast infections, steroids creams, nothing helped me. There was two times when the infection was so severe that a tight ring formed around my penis. In other words my penis didn't look healthy at all. I saw many doctors, spent a lot of money and like I said I ended up having a circumsicion as recommended by two urologyst. Your problem may not be the same as mine or not so severe. When I went to have the surgery I had some redness and when I removed my bandages, the balanitis was gone. My surgery was done one year ago and my penis still look very healthy, no signs of infections.  Why don't you send me messages to my inbox so I can explain more cus I really want to help you. dude.. i have been informed about circumcision but the dermatologist is saying it is the last step if all solution go in vain.
And i was diagnosed in early stage of balantis... what were your situation???
and what are the drawbacks and associated problem of circumcision??

Make an appointment with an urologyst cus your balanitis is causing you phimosis, it is like a tight ring. Also balanitis is a fungal or yeast infection. I had the same thing and it last it for 1.5 years. I went to many doctors, they prescribe me all kind of medicines and creams, I used home remedies, nothing helped me until two urologyst told me that I needed circumsicion cus my foreskin was giving me more problems that benefits. After thinking about it for a long time I decided to go for the surgery, I did it and all my problems solved with the removel of my foreskin. I still think that circumsicion has to be done only for medical reasons. See the urologyst. Try this home remedies to see if they work for you:
Soak your penis in a glass of water with two teaspoons of white vinegar twice a day.
after shower air dry your penis.
spread plain yoghourt in your penis once daily.
