Cramping with Diarrhea poo yellowish

Hello Dr. I am a 50 year old male, 6ft. I am over weight, and have been all my life,  260lbs.  I have no known health problems except to be obese. I had a physical one year ago and all blood work (cbc) were in normal limits. I had my recommended 50yr colonoscopy done 4 months ago, and it showed numerous diverticula’s in the colon (runs in family). I was told by the Dr. to just add fiber to my diet and see him in 5 years for a new colonoscopy. Two months ago, I had a level 7 pain in the lower abdomen area that would come and go, during the day and night for two weeks.  I had diarrhea and gas pains, and my poo  had a very bad smell., for two weeks this went on. I stopped eating fiber, and went to a low residue diet and it helped lower the gas pains and cramping. I had continued issues and found out when I ate any dairy products (milk and cheese) lactose, I would get diarrhea with cramps and pains too. So I stopped all dairy completely. Now for over a month my stools have been yellow in color and lose. If I eat any milk or cheese product I get cramps, gas, and diarrhea, within 3 hours. Until 2 months ago I never had problems with lactose.  I had a glass of red wine with a plain lettuce salad the other night and within 3 hours major cramping and diarrhea again.  When I eat anything, I feel my stomach area, does not feel right and lots of gurgling and my abdomen feels uncomfortable and muscle cramping in the abdomen as the food digests. I went to a walk-in clinic last week and a Dr. there ordered an antibiotic (qty 15) Metronidazol, 250mg. to see if it is a possible parasite, but so far no changes. I am almost finished with the 5 day order and still feel no better. My poo has been pail yellow and soft still and has a bad odor. Dr. do you have any idea what might be going on, and what would you recommend at this point for a next step to get better. This issue is affecting my life now and I feel something is going on.  Thanks Eric

Repeating the colonoscopy can be considered to exclude colitis, infection, or any anatomical abnormalities.

If the test continues to be negative, sending the stool of to evaluate for malabsorption can be considered.

Blood tests to exclude celiac disease can also be done.

If the tests continue to be negative, consideration of antispasmodic agents or tricyclic antidepressants can be considered to treat irritable bowel syndrome.

These possibilities should be discussed with your personal physician.

Followup with your personal physician is essential.
