Severe Pain and no answer...

d
I am 22 yr old female with severe stomach discomfort which has got progressively worse from its appearance in Aug this yr whilst on business in New York.  Symptoms = sore throat, swelling of membranes in nose (to point where insides are ulcerated), pain on eating or drinking which continues down esophagus and into stomach which is also painful to touch.  Stools are blackish and have mucus in them and I seem to go within an hr of eating.  Extreme fatigue also a symptom, as is belching and gas and indigestion and heartburn (difficulty catching breath).  The pain is also bringing on a headache - particularly across sinuses.  My neck and around my throat is also sore.

I have a history of Anorexia (since 13) and although, eating ok now and have a stable weight (41 kilos)have at times been down quite low.  I have never abused laxatives, but in past yr or so have been able to stop being sick after meals sometimes (have never binged).

I have also a history of ameobic dysentry in 1996 from a safari in Kenya, which was treated by 2 successaive doses of antibiotics. At the time a endoscopy revealed slight inflamation, but this was put down to gastroentetitous caused by the infection.

Since then, this spring I has a colonscopy as I noticed blood in my stools.  The dr said I had a slight tearing at the anus, he said caused by possible straining and gave me cream for piles.

I am in a stressful job and the latest set of symptoms seem to have been triggered from a period in New York where (having just split up from my boyfriend) I was having very little sleep, was drinking and smoking to xs, and was also vomiting.  I am now resting and trying to eat healthy (but calorific) foods to keep my weight up and give me some energy, but I am so scared of the damage that may have been done. I really want to get my weight up a bit, and obviously have stopped being sick (even when I feel really nauseaus) and have cut alcohol and cigerettes out completely. I am under a specialist, but am really concerned.  The pain  is constant and worse than ever.  After all the stress over the years my family are obviously quite unsympathetic and after past investigations having not revealed anything sinister do not think theres much wrong.

Has anyone any ideas what is wrong ?....why the nasal inflamation , swelling and itchiness if it is a stomach problem ?
Are there any foods that would perhaps  cause less pain ?(at the moment it burns all the way down).

I have severe pain on the right side under my ribcage.  I have not felt good for a few months now.  The pain is becoming unbearable, especially while sitting for long periods or while standing or walking for long periods.

Dear D,
Thank you for your email.  It is very difficult to tie all your symptoms together with a single diagnosis.  The colonscopy has excluded problems of the large colon as the cause for your gastrointestinal symptoms.  It is conceivable that disease involving the small intestine and/or stomach is the cause for the  black stool , weight loss, and pain.  Common conditiosn such as esophagitis must also be excluded.  You should ask your doctor about the possible benefits of an evaluation of stomcha nd intestine.

I am uncerrtain of the significance of the sore throat and swollen mucous membranes.  If you still have these problems  gastrointestinal causes e.g. esophageal reflux should be excluded.  If there is no reflux, consider an evaluation by an ear, nose and throat specialist as well as tests to excluded collagen vascular diseases (always a consideration when multiple organ systems are involved)

Sorry.  I think I make a mistake.  Just ask your doctor about  * Barret *  and  * Helicobacter  Pilory * * H. Pilory * Suerior mesentery is not for you.  Sorry, that was for another person.
