Little rash on golden retriever puppy

I got a 3 month old golden retriever, has had his 2 first series of shot. Survived parvo, I got him back from the vet a month ago. He's been fine ever since. But 2 days ago we noticed that he had a little hair loss on his head with a little scab. Today it seems a bit bigger and there 2 little red spots! (See picture) He does scratch it a little. Maybe that's why he's loosing more fur and more scabs are appearing.
I'm just wondering if it's something I should be worried about.
He's full of energy, he played with my sisters dog all day.
In the morning his feces are fine but during the day it gets softer! But nothing horrible.
I know that we have poison ivy behind our garage (where we can't reach it) and he does go there!
Anyway...

I agree with tx silver - it could be the poison ivy or she could be allergic to something else.  You mentioned a change in her stools during the day - could she be allergic to something in  her food?  It could also be a result of having her vaccinations.  Our Siberian husky is allergic to her vaccinations and we have go through a whole process just to get her ready for them.

You do sound like a loving and caring "dog parent".

Txsilver's "remedy" for poison ivy was something I've never heard before but think I'll try  it.  I learn something new every time I come out here.

Bless your baby's heart!   I can tell from your post you are a very concerned and loving pet custodian.  
It is possible that the rash you are seeing is not related ti poison ivy and will clear but I feel compelled to beg you to remove the poison ivy from your yard and to do it organically since your dog plays in that area.  Here is what my googling found:

It seems dogs can get poison ivy rashes in area with little fur.  You can get rashes from poison ivy from petting your dog after he has played in an area contaminated with it.

There seem to be two ways to remove poison ivy organically.  The first is to dig it up, but since you wrote that you cannot reach it you might try the second way.  That is to use at least 20% (or higher) vinegar as a foliar spray to take out the leaves. As new leaves come out, spray them too. Eventually the plant will die from lack of photosynthesis. Please note, this is not the same vinegar in your pantry and is available at most garden centers and some feed stores.
