I would definitely bring the discharge to the vet's attention. It could be an infection caused by something minor, such as something that has worked its way underneath the sheath. Sometimes a piece of a twig or something small works its way under there and it irritates the penis to the point where a wound starts and it becomes infected.
Antibiotics are made to kill bacteria. Some antibiotics are known as broad spectrum antibiotics, meaning they work on many different types of bacteria. However, some bacteria are gram negative and some are gram positive, and it could be that the bacteria causing the ear infection are one type and the bacteria causing the penile discharge are another type. This would explain why the antibiotics he's taking for the ear are not working to clear up the discharge. He may need to also be put on an antibiotic to clear up the discharge or, probably a safer option would be to put him on a broad spectrum antibiotic that will take care of both problems at the same time.
I wouldn't wait too much longer to bring this to the vet's attention, however. He's probably feeling pretty miserable because of the ear infection and if the problem causing the discharge gets any worse he could really end up feeling lousy. Also, by waiting, you run the risk of the infection, if there is one, becoming much worse and really becoming difficult to get rid of. The vet can do a culture of the discharge and that will tell him exactly what he's dealing with so he can treat it appropriately.

Did you mention the discharge to the Vet? You have follow up for the ear issue? Maybe you need to see another Vet for a second opinion and, if you do, mention the discharge. Just because he is on antibiotics does not necessarily mean he is on the right one. Cultures may need to be done.

thanks for the comment, it is very heavy, we wipe it with baby wipes and within seconds its back and dripping out. he has not been neutered yet. |He is also suffering from an inner ear infection so is on alreayd on antibiotics. this is affecting his balance but he also seems to be getting worse by the day, today i had to help him in the car and had to lift him onto my bed.
Is he neutered? A yellowish pencil discharge on a male dog, called smegma, is normal. However, if it seems heavy and he is not himself, he could have an infection in the sheath, bladder, etc. wash and rinse that area off well and watch it to see if the discharge comes back, how fast and how heavy. Probably best to get a Vet check. He may need to be flushed and/or given an antibiotic.
