partially opened surgical site with stitches...what should I do to protect the area?

I had a mass removed from my dog that was sub-muscular; so she had to have 2 layers of stitches. She contorted enough to scratch the area and partially opened the site. The vet said to just keep it clean or we could have a skin graft. Is there anything special I can do besides the skin graft to help her heal properly. I am thinking this happened because the vet had the stitches atop one another on both layers, causing the top outer most layer to not have as good of a healing base.
