Tonsils

My husband went to ent doctor for a thyroid  nodule because the family doctor him to seehim. now the ent doctor wants to do a biopsy of the nodule which is 6mm. Also when he look down is throat he only could see the top of his tonsils. Now he tells him to come back  so he could put down a light down his throat  to see the bottom of his tonsils. Why couldn't he do that in the first visit. When we were living in another state my my husband went to the ent doctor to cleaned out his ears wax. We moved to another state he saw a ent doctor in Jan 2016 to get the wax out of his ears. Never said anything of his tonsils. The doctor in Jan 2016 left the practice. So when the first family told him he has to see a ent doctor for the thyroid nodule so when made the appointment he found out that the doctor he was in Jan 2016 left the practice. So they gave him another ent doctor . The new ent doctor couldn't see the bottom of his tonsils. So he told him to come back so he could put a light down his throat. Could someone  explain this me. I do not understand why he couldn't do it one visit.
