Yes please keep it clean and/or think about getting circumcised. I was raised by my mom but she knew from experience (with my dad) that all sorts of things can happen if you don't keep it clean. Of course I did not listen (she was a woman..what did she know about boy privates. How naïve we can be lol).
Men can actually develop yeast infection (Candidiasis) under the foreskin and urinary tract (I had a few cases of that). The foreskin gets affected the most. It gets all red and itchy and, if let go for over a few days, it can even cause the skin to look like its cracking (but not in a dry way). It gets very painful to urinate, especially having to pull the skin back and sex is out of the question. You are a virgin, which is the only thing that puzzles me about you condition (of course it could be something different all together).
My fiancée was getting a lot of yeast infections and that’s where we figured I had gotten it from. Men don’t naturally have Candida under the foreskin but if not cleaned often it can develop there. Taking antibiotics can also cause this or having a weak immune system from cancer treatment or autoimmune disease. We found using the same OTC ointment that women use, for yeast infection, worked great. The few times it got really bad I took Diflucan (prescription). Like most meds, like this, it gets into your system and is secreted out through soft membrane like the lining of your organs and urinary tract (and broken or irritated skin like affected foreskin).
I don’t know if it helped but I wasn’t so careful cleaning up after peeing just in case the pee had some more potent amounts of the Diflucan in it. Over washing the area, at this point, could be bad for the situation. Keep soap away for a few days but do rinse with warm water when showering. Also using the Monistat ointment I mentioned above is a good idea.
Since be extra careful, I haven’t had it in ears. I rinse it a few time a day. Even dabbing it with a wet paper towel will do. I also sweat there a lot so putting on deodorant throughout the whole area helps. I trim the hair down as short as possible and shave where I don’t think there should be hair. I think this gives things less area to grow in. Wear looser clothes and if not boxers than briefs that are one size two large helps. Trust me I wore tight jeans and bikini briefs for most of my “young man’s” life…big mistake.
...oh! and don't be afraid or embarrassed to go see a doctor. It’s your manlyhood! You got to treat it right and if anything weird is going on get it checked out right away. Doctors are happy to help and teach young guys. If you are young and don’t want mom to go or know details (we know how moms can fuss), get an uncle or friend of the family to take you and tell mom it’s a guy thing:)

Last line of my comment left out a word. It should read: "And ALWAYS keep your foreskin CLEAN; otherwise you leave yourself vulnerable to irritations as well as disease." Good luck. Dr. J

Hi there. There are many reasons why your foreskin and/or shaft can become inflamed--some are just the result of not washing, sweat, irritation, etc., but some conditions may require you to see a urologist, so please re-post this question in the urology "Ask a Doctor" forum. And ALWAYS keep your foreskin; otherwise you leave yourself vulnerable to irritations as well as disease. Dr. J
