Eye bag small eye than other

About 10 years ago I got hit in my left eye and got a cut on the bottom side eye lid away from the nose, my eye was swollen for 2 weeks at the most. I slept on the left side on my bruise and cut because thats the side I sleep on and after the bruising went away and my cut shut now it has a scar, I noticed my eye seems smaller and a puffy bag, it wont and it hasnt gotten away since, it feels like under the eye ball something is pushing my eye up a little and making it uncomfortable, I never had this problem before I got hit and my right eye looks bulgy which is normal. Can it be a nerve that got damaged? Or is it fat tissue that my eye got pushed a little upward from sleeping on the side while my eye was healing? Or what can it be?
My left eye looks tired all the time and I have been to an opto twice and tells me am fine I can see, can a eye surgeon help me out?

Ok thank you, one more question, do I need permission by my Dr. in order to go see a oculoplastic sergeon? Because he dosent help much
