Eye exam results

Please help me understand these results.
VA     SC     20/400             W       OD  - 17.75 -100X 45   Add   +150
          CC     20/200                        OS  - 17.00-150X134

                                                        OS  N1                 TA  13  over 12       MI OU       Neo2.5  OU

Mitotic (-) ADD minimal reaction  2mm------3mm     Confrontation fields full-but poor fixation    
Color plates  does not appreciate stronger add.      In the box for abnormal comments by lens is 1-2 tms  or tns right side and 2 Tns on left side of the box. (anatomical position)

1). What questions should I ask and do you know of any help is to be had if we take him to Boston Ear and Eye hospital?   Thank you for your help

The -17.50 and -17.00 indicate very high myopia.  The other numbers (-1.00, -1.50) refer to the amount of astigmatism, and the 45, 134 refer to the orientation of the astigmatism.  The SC means vision without correction; CC means corrected vision, which should be 20/20 if the eye is anatomically normal.  The fact that the corrected vision is not 20/20 means that there is a problem inside the eye.  TA numbers are for the intraocular pressures, which are normal (13 right eye, 12 left eye).  TNS might be trace nuclear sclerosis, or slight yellowing of the normal lens of the eye.  I would have this person evaluated by an ophthalmologist; there may be some myopic retinal degeneration related to the high myopia.

Dear Dr. Fazio,
    Thank you and the other MD's for volunteering to man this site.  Without you all I wouldn't know how to help this person and I am already racked with guilt that I wasn't there all these years.  I was busy, working, studying and raising my own family, which by the way I raised intelligent, happy and well rounded adults for this world.  I will make an appointment in Boston on Monday.  Please don't mind me writing in again if I need definitions of medical jargon in the future.
