High Myopia with amblyopia-IOL\ICL surgery-PLEASE RESPOND.

Hello to all,

First of all sorry for my engl. skills, i'm from Romania.

I need a critical/crucial advise on what to do, regarding my sight quality.The resf of my life depends on it.
If it woth i will do it.

I will preffer to tell you, in a few words, about my condition.
-30 old male ( healty )
-High myopia from 4 years ( starting -3 ); now is -13 with 3.75 cyld. ( left and right )
-Born with squint in the left eye ( I have surgery done in 1990 ). This squint was gone for a wile but reappear over the years. If i'm not tired is not very visible. The eyes are not focusing in the right place.....
-Do to squint I have amblyopia in left eye. How I see with this eye ?
Well, the right eye is doing all the work, and the left eye is only "working" with right eye for peripheral vision ( I have better peripheral vision then central vision ). If i close the right eye ( dominant one ) and look with left I see better BUT I can not read the letters. I can not focus on a letter or a small object I take the object\letter "has it is-a brut image". Also, the field of vision is more reduce. I realised this on my own in the past years !! No doctor have told me this. Puting more dioptres did not help this eye. With a reduce prescription ( -10 and 2 cylder ) I see clear with this eye, better then right, but I lose the ability to focus and I tired fast.....

I think that quality of vision is somthing like this : glasses/contact/Laser surgery/ICL or IOL.
Is possible to do an ICL or IOL ? If the answe is YES:

What is the best option/safest for my ? ICL or IOL.
I wish to make the risk as minimal as possible, not to have a second intervention ! My father had cataract and i'm scared if I take the ICL ( VISIAN STAAR ) to do in 10 years cataract....this is why I tend for IOL.

If i coose the IOL, my option is for ZEISS AT LISA TRIFOCAL personalised lenses ( other options better ? ). I don't know how this IOL will interfere with the amblyopia. All my vision depends on the right eye.
1)Is it ok to take trifocal or better to have monofacal/bifocal ?
2)How this IOL will interfere with the amblyopia ?
3) How to customize the lenses for 2 eyes ? For example is ok to put trifocal in right and monofocal in left for distant vision? How is better/how to do ?
4) Is the amblyopia problem an advantage for me ? I think is much easy to fit and IOL because i do not have binocular vision and the left eye has a small contribution.
5)How do you think will be the adaptation of my brain to this change?. Expectation.


What is the wise choose for me : IOL or ICL for vision quality ?
**One doctor tells me to do ICL ( the natural lenses are OK ) and other tell's to do IOL.

If you wear a soft contact it will need to be a toric contact to correct your astigmatism. GP Contacts would probably give better vision but are often not as comfortable.  

You cannot get fit for contacts over the internet. You've worn soft contacts so you have an idea of the feel and vision. You will need to discuss this with the ophthalmologist or optometrist that is going to fit you.

There is no course of action that involves no risk. From least to most risk these are your options:  1. glasses  2. contacts  3. extraocular refractive surgery like PRK/Lasik 4. intraocular contact lens ICL 5. clear lens extraction also known as refractive lens exchange or if the nature lens has cataract a cataract/IOL (intraocular lens).

Even when done by a skilled experienced surgeon 4 and 5 carry risks of: infection, bleeding, cornea decompensation, double vision, droopy eyelid, retinal detachment, need for additional surgery, continued need to wear glasses, macular swelling, and small risk loss of eye.

A lasik done superficially PRK might correct 8-10 diopters of your RX depending on the thickness of the cornea and lasers available to you.

I would suggest you get a minimum of 2 opinions before you undergo ICL or cataract/IOL.

If you want to try and find someone click on the blue link their name creates and leave a message on their home page. By looking at this page you can tell if they are still active. If no posts in 3-4 years likely has wandered off the website for good

First of all there is no option you have that involves intraocular surgery (ICL and cataract/IOL-called a refractive lens exchange if no cataract) that is entirely safe.  Risks include infection, bleeding, cataract formation (ICL), corneal damage and retinal detachment, double vision and droopy eyelid.

Your safest option of course is glasses followed by (external) contact lens. Next would be lasik especiall PRK but its not likely all of your myopia and astigmatism will be corrected. however big difference between wearing -13 and -3 or 4.

The ICL is lower risk than IOL.  Important to note that neither is no risk. Consider getting at least 2 opinions prior to making your choice.

I think is more safe for me, for a moment, to put contact lenses. I never had contacts and I don't know how will be.

I need an adivse !

I have an examination and the doctor will put a Soft Contact ( PureVision ) for trial.

I see on the interenet that GP are much more better. Is true ?!?

Can you tell me exact name of the lenses, a brand... ?

BTW R: 12.75 with 3.5 cyl. and L is -11.50 with -3.5. I see 20/30 with my right eye.

Can you provide this information for me:  The best corrected visual acuity (e.g. 6/6  or english 20/20 rating) with glasses in each eye?

Good question !
Is hard to tell you because I do not rememeber ( I have an outdated prescription of 11.50, -3.50 L and R is 11.75 -3.50 that was OK). Now is -13 plus 3.75.

For the R I think with the best corection I can see 20/20 ( 80% percent sure ) and 20/40 (100%). For the L I think I can see the "big" E and 3 more lines but i wil strugle.
