What could cause these spots in my vision?

I see dark charcoal gray/black squiggly lines in my vision, there are probably about 10-12 lines and one slanted translucent blurry rectangle shaped spot in the very center on my vision. The image of the lines and the rectangle is the same in both eye's and occurs spontaneously multiple times per day for about 5min-4hours at a time. I have been to an ophthalmologist and was told he didn't know what was causing it because he couldn't see any defects in my eyes or abnormal pressure. Anyone else have this? does anyone know what it's called?

This is probably in your visual cortex and may be a form of migraine.
