light sensitive and glare

As you recall, I am a post RK patient of 1989 who had a crystalens HD implanted in 8/2008.  9/2008 I had the start of a detached retina in my upper RH corner which was successfully repaired.  A core vitrectomy was performed at the same time  I have no floaters now.  After the gas bubble disolved and I was off the drops, my pupil was remaining stuck open.  An adhesion from the 10:00 to 1:30 position was between the back of my iris and in front of the capsular bag.  The surgeon attributed it to me being on the atropine to long of a period of time or maybe the gas bubble was pushing the lens forward.  A posterior of the synchia was performed to break up the adhesion on 3/10/09.  I was on omnipred and nevanac up until 4/19/09.  The last visit 3/20/09  I still maintain my excellent visual acuity, but my IOP was 28 Right eye and in the non operated  left eye it was 16.  My retina and the vitreous is perfect according to the exam.

As of this day, I do not know what my IOP is and my next scheduled full exam is 5/11/09.  But to this day I can tell you that after a good night sleep and upon awakening after my cornea/eye is fully hydrated with my own tear production, I have the best quality of vision and as the day progresses, it feels like my eye starts to lose that moist refreshing feeling.  It doesn't affect my near as much as it does my distance.  During the day, I have to work the crystalens to see the sharp distance.  If I put artificial tears in my right eye, it sharpens the vision for a few seconds.

I am wondering if a pressure of 22 in an RK eye or any eye causes a little glare and loss of contrast and would a lower punctual plug help relieve some of these symptoms.

I wish I could just keep the best quality of vision that I have during the morning.  Any ideas.

They are safe, easy to place and help a lot of people. DO NOT get the so called SMART PLUG which is completely buried within the tear sac. It often causes irritation or tear sac infection and does not come out easily at all.

Thanks for addressing this for me.  Your input is always appreciated.  I just started with the flaxoil pill supplements.  I have seen a little improvement.  What are your thoughts on a lower punctual plug?   Thanks again

The IOP in an eye post RK is not reliable and to determine if you have glaucoma in the eye you need visual field test, optic nerve photos, corneal thickness, gonioscopy and OCT of nerve fiber layer.

your increase IOL could be due to the steroids you were on. If you IOP was elevated still then I would do a glaucoma work up.

As to your vision if it is in fact due to drying then using artifical tears throughout the day to keep the cornea hydrated should maintain the clear vision you have in early morning.

If tears keep it clear only a minute or two then you should be considered for Restasis therapy.
